浅谈CE方案治疗小细胞肺癌临床观察【摘要】 目的 探讨单用CE（卡铂-CBP；足叶乙甙-VP16）方案和CE与CAP（环磷酰胺-CTX；表阿霉素-THP；顺铂-DDP）方案交替治疗小细胞肺癌的疗效及生存期。方法 可供治疗的小细胞肺癌38例，其中女8例，男30例；年龄49~78岁，平均61岁；局限期10例，广泛期28例。Karnofsky 评分均在60分以上，38例随机分为治疗组20例和对照组18例。治疗组用CE方案（CBP：350 mg/m2，第1天；VP16：100 mg,1~5 d）；对照组用CE与CAP方案交替，CE方案同上，CAP方案为（CTX：350 mg/m2,第1天；THP：60~70 mg/m2,第1天；DDP：40 mg,1~3 d）同时碱化水化尿液，每3周为1个周期，2个周期为1疗程，所有病例均完成2个疗程，分别评价疗效和毒性，并随访生存期。结果 CE方案治疗小细胞肺癌比CE与CAP方案交替治疗的生存期长，疗效好，毒性反应小，无明显耐药现象。结论 CE方案是目前治疗小细胞肺癌的金标准，且对复发和转移的小细胞肺癌仍有较好的疗效，毒性反应轻，安全性好。 【关键词】 卡铂；足叶乙甙；小细胞肺癌；化疗 【Abstract】 Objective To dissuss the curative effect and survial period of the therapy on small-cell lung cancer by using CE regiment-only and the alternate regiment of CE and CAP. Methods Analysis on 38 cases of the small-cell lung cancer patients, including 8 female and 30 male aged from 49-78， median age 61,10 localization stadium, 28 generally stadium. The average score of karnofsky is over 60. The 38 cases are divided into two groups at random: treatment group has 20 cases and contrl group has 18 cases. We use the regiment of CE(CBP：350 mg/m2，the first day；VP16：100 mg,the first day) on the treatment group and the alternate scheme of CE and CAP on the control group. The regiment of CE is the same as above， but the regiment of CAP is (CTX：350 mg/m2,the first day；THP：60~70 mg/m2,the first day；DDP：40 mg,1-3 days) and we need to alkalify and hydrate urine at the same time. We make 3 weeks a period and 2 accomplished 2 courses of treatment.Respectively evaluate the curative effect and the toxicity and examine the survival period at any moment. Results Compared with the alternate regiment of CE and CAP that is used to cure the small-cell lung cancer, the curative effect of the regiment of CE is better; the lifetime of regiment of CE is less. And we found that there is no drug fast obvionsly.ConclusionNow the regiment of CE is the gold standard to cure the small-cell lung cancer.We can even still get a good curative effect on the small-cell lung cancer by
