Methods Retrospectively analyzed the clinical data of 1 case involving the Rolandic area of lesional epilepsy and the relevant literatures were reviewed, the operation strategies, technical details, and operation results of the multimodality image functional neuronavigation combined with awaking anesthesia and intraoperative direct electrical stimulation in surgery for lesional epilepsy were analyzed and discussed. Results The total resection of epileptogenic foci was achieved in this patient in whom the neural function was saved. And there was no permanent complications and seizures. Conclusion The application of multimodality image functional neuronavigation combined with awaking anesthesia and intraoperative direct electrical stimulation in surgery for lesional epilepsy in Rolandic area can effectively protect the function of cortex and fasciculus, and control the seizure effectively.
