保留耳前鬓角形态并预防脱发颞颊部除皱术[摘要]目的:介绍一种保留耳前鬓角形态并且预防术后脱发的颞颊部除皱术式。方法:颞部切口位于发际缘后方5cm左右并与鼻唇沟平行,耳前切口位于鬓角后缘。颞部于颞浅筋膜深面剥离至发际缘,切开颞浅筋膜于其浅面剥离至眼轮匝肌浅面。耳前于表浅肌肉腱膜浅面剥离至咬肌前缘。垂直于颞部切口向外上方悬吊颞浅筋膜,垂直于鼻唇沟向外上方悬吊腮腺浅面的表浅肌肉腱膜。结果:2008年以来应用该法治疗颞颊部面部老化患者24例,效果满意,未发生严重并发症。结论:该术式保留了耳前鬓角的自然形态,术后切口瘢痕隐蔽,无明显脱发,避免了重要神经、血管的损伤。 [关键词]面部老化;多层次除皱术;鬓角;脱发 [中图分类号]R622[文献标识码]A[文章编号]1008-6455(2010)05-0632-03 A new technique of temporal and buccal facelift without configuration of sideburns destruction and baldness formation YAN Ying-jun1,QIAO Qun2,ZHAO Ru2,AI Hong-mei1,WANG Cong-feng1,ZHAO Zuo-jun1,LIANG Wei-zhong1 (1. Department of Plastic Surgery, Peking Union Medical College Hospital, Beijing 100032, China) Abstract:Objective To introduce a safe rhytidectomy technique without configuration of sideburns destruction and baldness formation. Methods The operation was performed with temporal incision paralleled to nasolabial sulcus about 5cm behind hair line and preauricular incision at the rear edge of sideburns. The temporal flap was dissected under superficial temporal fascia until to hair line. Then superficial temporal fascia was splitted and the temporal flap was dissected above superficial remporal fascia until superficial to orbicularis oculi muscle. The preauricular flap was dissected superficial to Superficial Musculo-aponeurotic System(SMAS) until to the front edge of masseter muscle. Superficial remporal fascia was suspended vertical to the remporal incision and SMAS superficial to parotic gland was suspended vertical to nasolabial sulcus. Results 24 patients with temporal and buccal aging face were treated using this technique since 2008. All patients reported fine rejuvenescent effects without any severe complications. Conclusion This operarion technique may maintain natural configuration of preauricular area and sideburns, result masked incision scar together with no obvious baldness, and avoid important nerve
