（编号：201500317） 基层医院颈部小切口在甲状腺切除术中应用可行性分析 陈志山 洪黎清 杨昌毅 傅少龙 作者单位：362300福建南安市医院普外科 Tel Email:chenzhishan_nn@163.com 【摘要】 目的 探讨基层医院颈部小切口在甲状腺良性肿瘤手术中的疗效及安全性。方法 总结我科2010年2月至2014年1月期间对248例甲状腺良性疾病行甲状腺切除术的患者，其中传统手术62例，颈部小切口186例，分析两组患者的手术时间、出血量及并发症发生情况，进一步说明颈部小切口在甲状腺良性疾病中应用的可行性。结果 小切口组前20例与传统组手术时间（min）比较为70±17.1 vs.57±19.3，P＜0.05，小切口组后166例与传统组手术时间（min）比较为51±22.2 vs.57±19.3，P＞0.05；小切口组与传统组手术出血量（ml）比较13±6.6vs. 12±4.3，P＞0.05；两组喉返神经损伤各1例，传统组甲状旁腺功能减退1例，两组均无喉上神经损伤、术后出血及短期复发。结论 颈部小切口手术方式在甲状腺切除术中有良好效果，不增加手术并发症，具有切口小、达到美容效果，适合在基层医院推广。 【关键词】 甲状腺； 小切口； 微创 The application with Mini-Incision thyroidectomy in Primary Hospital Chen Zhi-shan, Hong Li-Qing, Yang Chang-yi, Fu Shao-long. Department of General surgery,the Hospital of Nan,an City,Nan,an 362300,Fujian Province, China. 【Abstract】 Objective TO explore the Efficacy and Safety with Mini-Incision in thyroid surgery .Methods TO summary the 248 cases of thyroid benign diseases thyroidectomy patients in our department from February 2010 to January 2013, including traditional surgery 62 cases and 186 cases of cervical incision. By analyzed the two groups in operative time, blood loss and complications ,and to illustrate the Mini-Incision in the neck benign thyroid diseases feasibility of the application. Results Compared the Mini-Incision group in the begin 20 cases with the traditional group of surgery time(min) is 70±17.1 vs.57±19.3，P＜0.05,but the small-incision group in the after 166 cases with the traditional group of surgery time(min) is 51±22.2 vs.57±19.3，P＞0.05. The Mini-Incision group and the traditional group the blood loss(ml) are 13±6.6vs. 12±4.3, P0.05. Mini-Incision group and the traditional group each have one case of recurrent laryngeal nerve injury. The hypoparathyroidism has one case in traditional group. There were no laryngeal nerve injury, postoperative bleeding and short-term recurrence in the two groups.
