关节镜下清理术治疗25例髌前滑囊炎临床研究.DOCVIP

改进关节镜下囊内切除治疗难治性髌前滑囊炎30例 傅要武1 陈宗建1 吴有坤1 胡小军2* (1.重庆市荣昌县中医院，重庆 荣昌 402460；2.重庆市荣昌县人民医院骨科，重庆 荣昌402460) 摘 要 目 的:探讨改进关节镜下囊内切除治疗难治性髌前滑囊炎的疗效。方 法: 自2009年6月-2014年5月对我院30例复发髌前滑囊炎患者，男22例，女8例。病人平卧，硬膜外阻滞麻醉，囊肿旁开口,吸出囊液,注入灌注液，关节镜下用刨削刀将增生肥厚的滑囊后壁予以完全刨削切除,直至见到正常组织；滑囊前壁的只做适当刨削，切除台癣样增生滑膜组织，防止皮肤破裂。囊腔内彻底冲洗干净 ,弹力绷带加压，术后固定2周。结 果:术后随访，全部患者均无感染、复发；有1例患者残留有膝前疼痛，1例患者局部皮肤有麻木感；其余无肿胀，关节活动正常。结 论:改进的关节镜下治疗髌前滑囊炎，疗效更好、防止了皮肤并发症，更适宜临床开展。 关键词： 髌前滑囊炎; 改进关节镜; 囊内切除 中图分类号: R816.8 文献标志码: A Arthroscopic Intracapsular ResectionTreatment of 30 Cases Refractory Prepatellar Bursitis Fu Yaowu1 Chen Zhongjian1 Wu Youkun1 Hu Xiaojun2* (1.Rongchang County Hospital of Traditional Chinese Medicin in Chongqing , Rongchang, Chongqing 402460;2. Department of orthopedics of Rongchang County Peoples Hospital in Chongqing Rongchang, Chongqing 402460) Abstract Objective: To investigate the efficacy of the arthroscopic intracapsular resection treatment of refractory prepatellar bursitis.Methods:From June 2009 to May 2014 in our hospital, 30 cases of recurrent prepatellar bursitis patients(male 22 cases, female 8 cases). patients prostrate, epidural block anesthesia.cyst side was opened, cyst fluid was extracted and perfusion fluid was injected.Under arthroscopic free particles, adhesion, fiber cables were thoroughly removed with shaver.Pyknosis and hemostasis were achieved with a RF gasification electrode knife. Sac was thoroughly rinsed,pressure bandages were used,postoperative knee extension was fixed for 2 weeks.Results: Postoperative follow-up, all patients have no infection, recurrence. One patient had residual anterior knee pain. One patient with local skin numbness. No other swelling and joint activities are normal and all patients had no recurrence and infection.All patients were shown to have anterior knee skin depressions in different degrees, no local residual scar,no joint swelling, and joint activities were normal.Conclusion:Arthroscopic improvement treatment of prepatellar bursitis
