医学临床研究 20l0年 8月 第 27卷 第 8期 JClinRes，Aug．2010，Vol27，No8 · 论著 · 冠状动脉粥样硬化性心脏病的外科治疗 王晋 封加涛 (广东省佛山市第一人民医院心血管外科，广东 佛 山 528000) [摘要]【目的】总结应用 364例冠状动脉旁路移植手术(CABG)治疗冠心病的临床经验。 【方法]364例 冠心病 中男302例 ，女62例 。年龄 34～82(64．4±l6．8)岁。体重38～89(65．4±21．3)kg。46例在常温心脏 不停跳下进行CABG，318例在全麻低温体外循环下进行 ，经升主动脉灌注含血冷停跳液保护心肌 ，并于循环 心脏跳动下选择好冠状动脉切开部位 ，若有合并病变存在(如室壁瘤、二尖瓣关闭不全等)需先处理，然后再 行 CABG。用左 内乳动脉 326条 ，大隐静脉 764条，蛇形静脉桥 108条 ，平均 3条 。 【结果]354例 患者痊愈 出 院，心绞痛完全缓解 。l0例术后 因低心排死亡 。316例随访 2个月至 6年 已恢复正常生活或工作 。 【结论】 CABG是治疗冠心病 重要 而有效的方法。 [关键词] 冠状动脉疾病 ／外科学； 冠状动脉硬化 ／外科学 SurgicalTreatmentofCoronaryHeartDisease WANG Jin， FENG Jiatao (Cardiovascular SurgicalDepartment， PFirstPeople’SHospitalofFoshan，Foshan ，C-uangdong528000，China) [Abstract]【()bjective】T0summarizetheexperiencesofcoronaryarterybypassgrafting(CABG)intrea tingcoronaryheartdiseasepatients．[Methods]Amongthe364coronaryheartdiseasepatients，302patients weremaleand62patientswerefemaleagedfrom 34～ 82yearswiththeaverageof64．4± 16．8years．Body weightwas38-- 89kgwiththeaverageof(65．4土21．3)kg．CABG wasperformedin46casesatnormalbody temperaturewithbeatinghearts．Therest318underwentCABG underhypothermiageneralanesthesia．Cardi acmusclewasprotectedwith infusionofbloodcardioplegiafrom ascendingaorta．Siteofincisiononcoronary arterywaschosenwhileheartwasstillbeatingatthestageofparallelcirculation．Disorderssuchasventricular aneurysm andmitralinsufficiencyshouldbetreatedbeforeCABG．Forthisgroupofpatients，326leftinternal mammaryarteries，764 saphenousveinsand 108 sequenceanastomoseswereused．Average3 bridgeswere doneperperson．[Results]The354patientsdischargedfullyrecoveredcompletelyfreeofanginapectoris．Ten patientswithCABG diedoflow cardiacoutputafteroperation．FolloW—uphadbeendonein3l6patientsfor2 monthsto6years，andallofthemrecoveredtonormallifeorworking．[Conclusion]Coronaryarterybypass graftingisan importantandeffect
