精品论文 参考文献 弹性成像在甲状腺小结节中的价值 南文会　郭洪山 天津市西青医院耳鼻咽喉头颈外科　天津　３００３８０ 　　【摘要】　目的　探讨弹性成像在甲状腺小结节中的价值.方法　２０１２－２０１４年于我院行手术治疗的５２例直径le;１０mm 甲状腺结节患者,共６３个结节,所有患者术前均行常规超声和弹性成像检查,以手术病理结果为诊断金标准,比较常规超声和弹性成像诊断直径le;１０mm 甲状腺结节良恶性的敏感性、特异性和准确性.结果　常规超声诊断直径le;１０mm 甲状腺结节良恶性的敏感性、特异性和准确性为６８．２９％、５４．５５％、６３．４９％;弹性成像诊断直径le;１０mm 甲状腺结节良恶性的敏感性、特异性和准确性为８５．３６％、７７．２７％、８２．５４％.弹性成像诊断直径le;１０mm 甲状腺结节良恶性的准确性明显高于常规超声,且差异有统计学意义(chi;２＝５．８０,P＜０．０５);对于直径le;１０mm 甲状腺结节良恶性诊断的敏感性和特异性,弹性成像虽高于常规超声,但差异无统计学意义(P均＞０．０５).结论　弹性成像可以提高直径le;１０mm 甲状腺结节良恶性诊断的准确性,有着广阔的临床应用前景. 【关键词】　弹性成像;　甲状腺小结节Thevalueofelastographyinthyroidmicronodules　WenhuiNan HongshanGuo　(Xiqinghospitalintianjin )【Abstract】Objective　Toinvestigatethevalueofelastographyinthyroidmicronodules．Method:　Fromyear２０１２to２０１４,therewere５２operationtreatedpatientsofthyroidnoduleswithdiameterlessthan１０mm．Thereweretotally６３nodules．AllthepatientsunderwentcommonultrasoundandelasＧtographybeforeoperation．Thediagnosisofpathologywerethegoldstandard．TheaimweretocomparewithcommonultrasoundandelastographyindiＧagnosisofthethyroidnoduleslessthan１０mmindiameterwithsensitivity、specificityandaccuracy．Results:　CommonultrasoundinthediagnosisofbeＧnignandmalignantthyroidnoduleslessthen１０mmindiameterofthesensitivity、specificityandaccuracywere６８．２９％、５４．５５％、６５．０８％;IntheelastogＧraphygroup,theywere８５．３６％、７７．２７％、８２．５４％.Theresearchshowedthattheaccuracywashigerintheelastographygroup,andtherewassignificantstatisticaldifference(chi;２＝５．８０,P＜０．０５);thoughthesensitivityandspecificitywerehigerintheelastographygroup,therewerenosignificantstatisticaldifference(P＞０．０５)．Conclusion:　Theuseofelastographycanimprovetheaccuracyofdiagnosisthebenignandmalignantthyroidnoduleslessthan１０mm【Kienydiwaomrdeste】r,andtherewillbeapromisingprospectsinclinicalapplication． elastography;　thyroidmicronodules 【中图分类号】R５８１ 【文献标识码】B 　　【文章编号】１００８－６３１５(２０１５)１０－０５８７－０２　　 　　甲状腺微小癌(thyroidmicrocarcinoma,TMC)是指癌灶直径le;１０mm 的甲状腺癌,一般无明显症状,大多数为偶然发现,对于le;１０mm 的甲状腺结节如何处理,临床上存在很大争议,其良恶性的判断直接关系到临床处理方式的选择.超声虽然是甲状腺结节的首选检查方法,可以提高甲状腺结节的检出率, 但对于le;１０mm 的甲状腺
