Conclusion Look at the picture dialogue education mode in community diabetes health education has important significance. 　　【Key words】Diabetes education　 Educates the tool Map-read dialog Diabetes patients in Community Chronic Disease Management 　　在本社区的慢病管理工作中，发现糖尿病患病率逐年上升，这不仅给患者及家庭带来沉重的家庭负担，也给社区的慢病管理工作带来了较大的压力。本社区地处城乡结合区，患者的文化程度相对较低，这给糖尿病教育带来难度。众所周知糖尿病健康教育是整体治疗护理的重要环节，是糖尿病治疗的“五驾马车”之一，搞好糖尿病教育有着非常重要的意义。因此本社区对辖区内部分糖尿病患者运用“看图对话”教育模式进行有效的健康教育，现报道如下： 　　对象及方法： 　　1 对象 　　根据WHO糖尿病诊断标准,选择2012年1～3月本社区居民健康档案中的糖尿病患者共72例,72例采用随机数字表法分成试验组和对照组各36例。两组在性别、年龄、身高、体重、文化程度、认知、病情等方面的差异均无统计学意义（Pgt;0.05），具有可比性。入组的患者生活能够自理，行动自如，排除神志不清、明显智力障碍、严重精神病、截肢、心肾功能不全或多器官衰竭患者。其中男42例,女30例;年龄56～79岁
