ABsTRAcTobjective:T0evaluatetheoutcomeandpostoperativereactionofduralsubstitute(Neuro—
patch)applyinginthetreatmentofChiariImalformation(CMI)associatedwithsyringomyelia(SM).
Methods:FortvpatientsofCMIassociatedwithSMwereoperatedinourdepartmentfromJu1.2002to
Ju1.2004.A1lpatientsunderwentposteriorcranialfossadecompressionandduraplasty.Theyweredivid—
edintotwogroups.20patientsbeingrepairedwithNeuropatch(Neuropatchgroup),andtheotherswith
aHto1ogousfascialata(fasciagroup).Therewere6malesand14femalesinNeuropatchgroupand10
malesandl0femalesinfasciagroup.Theoperationswereperformedundergeneralanesthesiaviasuboc—
cipitalapproachandtheextentofposteriorcranialfossadecompressionrangedfrom20cnl(5cnl×4
cn1)to35cnl(5cnl×7em),Therenloxralofposteriorarchofatlasdependedontheextentoftonsillar
herniation.andthedurawasopenedi11shape.TheNeuropatchwascutintotriangularshape,andthe
samesize({autologousfascialatawasuse(1infasciagroup.Thepatchesweresuturedtightlytothedura
matterineachgroup.lheincisionwas(_losedlayerbylayeranddrainagewasused.ifnecessary.Antibi-
oticsandhormonewereroutinelyused.Fhedurationofoperation,postoperativefeverwereevaluated,the
outconleoftheoperationwasevaluatedbyTatorscale,andthedatawereanalyzedwithstatisticsoftware
SPSSl0.0.Results:Therewerel2patients(60%)whosufferedfrompostoperativefeverintheNeuro—
patchgroup,and9patients(45%)inthefasciagroup(x=0.902,P=0.342).Seventeenpatientsin
eachgroupwereimprovedpostoperatively.Thedurationofoperation,postoperativefeverandantibiotics
usedwerecomparedbetweenthetwogroups.Nosignificantdifferencewasfound,butthedurationof
postoperativefeverandthetimeofhormoneusedwerediffe
