We take confidentiality of our patients very seriously so we need to be assured of your identity before releasing any information to you. The details of what we need to see are at the back of the form. We are entitled to make these checks under Section 7 (3) of the Data Protection Act 1998. Please note that we may notify the police in the event of a fraudulent application. The Data Protection Act 1998 requires us to process your request within forty days from the time we receive the application, including your proof of identity.
