Olivia (7) Answer: Offer Olivia a second bolus dose of a strong opioid. Continue to assess the effectiveness of pain relief using an age-appropriate pain scoring tool every 30 minutes. Although Olivia’s pain begins to subside, 1? hours later her pain is again classified as ‘severe’. What would you do now? Olivia (8) Answer: Consider patient-controlled analgesia because repeated bolus doses of a strong opioid are needed within 2?hours. After 2 days of patient-controlled analgesia, Olivia’s acute painful sickle cell episode has resolved and her pain has significantly subsided. What do you need to do to continue to manage Olivia’s condition until discharge? Olivia (9) Answer: Decrease the frequency of assessing the effectiveness of pain relief to at least every 4 hours. Continue to be aware of possible acute complications. Follow locally agreed protocols for managing acute painful sickle cell episodes to step down pharmacological treatment.
