摘 要 目的：探讨耳鸣从急性向慢性发展的中西医机制，从耳鸣患者的负面情绪加重为出发 点，着重论述情绪对耳鸣的维持和发展过程所产生的作用，以及中医“治神”对耳鸣 治疗的意义，提出耳鸣“形神共治”方案，并进行临床试验同传统辩证方案进行比较， 以此来分析本方案临床疗效和优越性。 方法：纳入90 例耳鸣患者，分为针刺组（30 例），情绪调节组（30 例），联合刺激 组（30 例）。使用耳鸣程度量表和正性负性情绪量表（PANAS）分别记录和评价被试 在基线期，第2 周，第4 周的耳鸣严重程度和负性情绪水平。 结果：第 4 周联合刺激组的耳鸣严重程度均比针刺组和情绪调节组要低，而负性情 绪水平要比其他两组要低。对联合刺激组第 4 周的耳鸣严重程度和负性情绪水平进 行逻辑回归分析，结果显示两者呈负相关关系。 结论：“形神共治”治疗要明显优于单纯的“调形”和“调神”治疗；耳鸣的严重程 度和负性情绪存在正相关关系，即负性情绪水平越低越有利于耳鸣程度的下降，这为 “耳鸣-负性情绪-耳鸣加剧”恶性循环的存在提供了间接证据，为耳鸣治疗提供了新 的治疗靶向。 关键词：针刺 调神 耳鸣 Clinical research on effect of acupuncture and Governing spirit treatment on tinnitus Abstract Objective: Based on the tinnitus of the acute-to-chronic development of Chinese and Western medicine mechanism, with the negative emotions of patients with tinnitus as a breaking point, to discuss how the emotions ( particularly negative emotions) affect tinnitus in the maintaining and development ,and application of Governing God theory in traditional Chinese medicine in treatment of tinnitus. A treatment method of combination with form and spirit for tinnitus is proposed and then compared to the traditional dialectical programs in clinical trials, in order to analyze the clinical efficacy and superiority of this method.
