减少胎膜早破孕妇羊水流出的新型充气装置的临床应用 【摘要】目的 探讨臀部充气装置用于胎膜早破孕妇的临床效果。方法 将180例胎膜早破的孕妇随机分为观察组92例，对照组88例，观察组采用臀部充气装置抬高孕妇臀部，减少羊水流出;对照组采用头低脚高位，抬高床尾15°～30°，以减少羊水流出。结果 观察组孕妇卧位的舒适度、羊水流出量与对照组相比有统计学意义(P0.05)，观察组剖宫产、顺产及助产率与对照组比较差异有统计学意义(P0.01),两组在早产、胎儿宫内窘迫、新生儿窒息方面有统计学意义(P0.05)，但在产褥期感染率方面无统计学意义。结论 臀部充气装置用于胎膜早破孕妇有显著效果，值得在产科临床应用。 【关键词】 胎膜早破;臀部充气装置;应用 [Abstract] Objective To investigate the buttocks inflatable devices for the clinical effects of pregnant women, premature rupture of membranes.Methods 180 cases of premature rupture of the pregnant women were randomly divided into the observation group 92 cases,88 cases of the control group, the observation group was treated with hip inflatable device elevation in pregnant women hips, reduce the outflow of amniotic fluid; the control group using the Trendelenburg position, raise the bed tail of 15deg; to 30deg;, to reduce the outflow of amniotic fluid.Results The observation group maternity decubitus comfort, the outflow of amniotic fluid volume compared with the control group had statistical significance (P0.05), the observation group cesarean delivery, birth and midwifery rate compared with the control group the differences were statistically significant (P0.01), the two group in preterm labor, fetal distress, the palace had statistical significance in neonatal asphyxia (P0.05), but in the puerperal infection rates showed no statistical significance.Conclusion Hip inflatable devices for women with premature rupture of membranes has a significant effect, is worth applied in clinical obstetrics. [Key words] Premature rupture of membranes; Hip inflatable device; Clinical application 胎膜早破指在临产前胎膜破裂，发生率约占分娩总数的2.7%～17%[1]，是产科常见并发症。其主要危害是有羊水流出时，易引起脐带脱出到阴道内引起脐带受压，胎儿缺氧，引起新生儿窒息，导致宫内及产后感染率升高，诱发早产。减少羊水流出，防止脐带脱垂，预防感染成为胎膜早破孕妇护理的关键，因此，对2009年1-12月对胎膜早破的孕妇使用了我们自行设计臀部充气装置，收到良好效果，现报告如下。 1 资料与方法 1.1 研究对象 选择2009年1-12月胎膜早破孕妇的产妇680例，选择年龄在21～40岁,平均产龄25.3岁,孕周27+3～42+2周，无其他妊娠合并症的孕妇180例，随机分两组，观察组92例，对照组88例。 1.2 材料与制作 臀部充气装置时由厚度为3mm的软质硅胶片，经裁剪粘合做成女性扁便盆形，在臀部充气装置一侧有充气管，充气管与臀部充
