Delayed-Release) in the Treatment of Psychosis with Schizophrenia The pathophysiology of schizophrenia has been hypothesized to involve a disturbance in the gamma amino butyric acid (GABA) system GABA can regulate mesolimbic dopaminergic activity, linked to the symptoms of schizophrenia1 Divalproex sodium, through a GABA-ergic mechanism inhibiting the action of dopamine, may reduce psychotic symptoms1 Divalproex Use inSchizophrenia Patients and Methods A 4-week, randomized, double-blind, parallel-group, multicenter trial 2 treatment groups Monotherapy Olanzapine (OLZ), n=65 Risperidone (RISP), n=60 Combination therapy Divalproex (DVPX) and OLZ, n=66 DVPX and RISP, n=58 Patients with schizophrenia, 18 to 65 years old Acute exacerbation of psychosis associated with schizophrenia: Recent or current hospitalization PANSS Total Score ? 60 (1 to 7 scale) Minimum score of 8 on any 2 of the 4 items from the psychosis cluster of the BPRSd A total score of ?6 on one of the following pairs of BPRSd items Hostility and uncooperativeness Excitement and tension A positive response to antipsychotics in the previous 2 years Exclusion Criteria Current diagnosis of schizoaffective disorder, drug-induced psychosis, manic or major depressive episode Hospitalized for approximately greater than 2 weeks for current episode at the screening visit More than 3 psychiatric hospitalizations in the previous 6 months or had more than 8 weeks of psychiatric hospitalization in the previous 12 months Urine drug screen positive for cocaine, phencyclidine (PCP), opiates, or amphetamines History of substance or alcohol abuse/dependence within 1 month prior to the screening visit Antipsychotics titrated during the first 6 days to fixed daily doses of 6 mg risperidone qhs or 15 mg of olanzapine qhs Divalproex sodium delayed-release (DR) initiated (Day 1) at 15 mg/kg/day bid and titrated at the physician’s discretion to a maximum of 30 mg/kg/day
