Wisconsin Office of Rural Health 2013-2014 Rural Community Grant Proposal Guidelines The Wisconsin Office of Rural Health (WI-ORH) expects to receive financing through the federal Rural Hospital Flexibility Program to fund efforts to support rural health improvement projects, including the Rural Community Grant program. This program provides funds to rural hospitals and healthcare coalitions to assist them in addressing health issues in their communities. Over the past 10 years WI-ORH has funded over 60 community projects. During this time the program has evolved to meet changing funding requirements, in response to changes in the rural healthcare environment and to improve the program. Current program highlights: Planning and implementation grants: This year we will offer both planning and implementation grant options. If your coalition receives a planning grant and successfully achieves your project goals, you will receive implementation funding in 2014-15; you will not compete with other applicants. This is essentially a two year grant but will be dependent on WI-ORH receiving federal funding and your submission of an implementation work plan, budget, logic model and evaluation plan next year. This year maximum planning grants will be $5,000 and implementation grants will be $15,000. Evaluation and outcome measurement: Grant funders, especially federal funders, are increasingly interested in seeing quantitative results of funded programs. This includes measuring program activities (how many meetings held, how many people participated in class, etc.) but especially measuring changes that occurred. This could include weight loss, lower blood pressure, an increase in volunteer EMTs or any change in behavior as evidenced, for example, in follow up surveys.
