* * * * * * Fecal electrolytes and the fecal osmotic gap. The osmolality of colonic contents is in equilibrium with body fluids and is approximately 290 mOsm/kg. The total concentration of electrolytes, therefore, cannot exceed 290 mmol/L. In secretory diarrhea, almost all of the osmotic activity of colonic contents is due to electrolytes, and thus, the estimate of electrolyte content (2 × ([Na+ ] + [K+ ])) is almost 290 mmol/L.
