NQSW Outcome Statement Matrix Grid Guidance: This matrix tool replaces Section Five, in the Skills for Care red resource pack to help you in the process of collecting evidence to go towards your portfolio. It can be used in supervision with your supervisor to identify and record activities and outcomes met. It is a quick and easy reference document.
