后牙即刻种植组织处理 　　[摘要]目的：通过对后牙即刻种植的临床病例的观察研究，分析评价后牙区即刻种植手术过程中手术区组织处理的重要性。方法：通过对后牙区即刻种植的123个临床病例进行观察研究，对后牙区采用适当的处理技术。结果：术后6个月后，患者均不出现炎症，具有正常咀嚼功能，骨质愈合生长较好，已经全部完成骨性结合，组织也都与正常状态相似，患者对治疗都表示满意。结论：在后牙区即刻种植手术过程中，正确的软硬组织处理不仅可以缩短手术时间，还可以较好的恢复缺失牙周围的骨组织以及周围组织的形态，会达到更好的美学要求。 　　[关键词]即刻种植；后牙；组织处理 　　[中图分类号]R783 [文献标识码]A [文章编号]1008-6455（2014）07-0566-04 　　 　　Exploration of the tissue treatment after immediate planting of back tooth 　　GUO Li-juan 　　(Department of Stomatology,Suzhou EENT Hospital,Suzhou 215006,Jiangsu,China) 　　 　　Abstract: ObjectiveTo analyze and evaluate the importance of surgery operation on tooth area after planting immediately through the observation of clinical cases after the teeth planting immediately. ResultsSix months after surgery, inflammation and abnormal masticatory function were not found. Patients became better in bone healing and tissue condition that they were satisfied to treatments.ConclusionDuring immediate planting operation process of the back teeth, the right tissue treatment can not only shorten the operation time, also can be better restored lost bone tissue around the teeth and the formation of surrounding tissues. It can achieve better aesthetic requirements. 　　Key words:immediate planting;back tooth;tissue treatment 　　近几年来，修复牙列缺失或缺损的最佳治疗途径是口腔种植。而口腔种植中的即刻种植有着更大的发展前景。即刻种植是指缺损牙齿被拔除的同时就可以立即把牙根代替体植入牙槽窝，这样不仅使得缺牙期间牙槽骨骨组织的丧失和弱化程度减少，通过对重新建立牙槽骨的形态和功能的早期恢复，可以减少患者手术次数以及手术过程中的痛苦，同时可以让牙龈组织外形达到最佳美学效果[1]。但拔牙后牙窝洞口软组织的缺乏和硬组织的损伤，常导致手术中难以关闭创口或者关闭创口有张力，这样严重影响了手术过程中种植体周围软组织，使其处于不稳定状态，也不能很好的达到美学要求，严重时可能会使种植体的骨性结合不能完成。随着患者对美学效果期望值的提高，手术周围组织的处理技术不可忽视，成为即刻种植治疗其中的一个关键步骤，笔者通过对后牙即刻种植案例的调查分析手术期间周围组织的处理的重要性。 　　 　　1资料和方法 　　1.1一般资料：选择2012年7月～2013年11月在我科种植中心进行后牙即刻种植的患者共 123例（男53例，女70例）即刻种植病例，年龄22～80岁，平均(52.43±7.31)岁。这些患者中外伤有13例，残冠根101例，也存在交叉病例。 　　1.2 病例纳入标准：①拔除的牙齿是不能保留下来的患牙或是不能得到治愈的病牙，这些牙齿大多数是由于外伤及不良修复而造成根折或者残根残冠；②患者没有急慢性炎症存在，接受即刻种植的区域的周围组织无明显炎症；③牙槽骨没有明
