128层螺旋CT对胃癌的诊断价值分析 　　[摘要] 目的 探讨128层螺旋CT对胃癌的诊断价值。 方法 收集90例胃癌的临床资料，患者均接受128层螺旋CT检查，术后均进行病理检查。以术后病理诊断为标准，统计分析CT对胃癌及其TNM分期的准确率。 结果 128层螺旋CT对胃癌的诊断率为97.8%，对早期胃癌的检出率为71.4%，进展期胃癌检出率为100%。T1、T2、T3、T4期的诊断准确率分别为71.4%、76.9%、86.7%、90.0%，N0期、N1期、N2期诊断准确率为76.0%、64.3%、75.7%，M0、M1期诊断准确率分别为92.5%、91.9%。 结论 128层CT可清晰显示胃癌的形态、浸润程度、淋巴结转移和远处转移等情况，结合多平面重建及CT仿真内镜等技术处理进行临床分期，利于胃癌诊断及治疗方法的选择。 　　[关键词] 128层螺旋CT；胃癌；TNM分期；远处转移 　　[中图分类号] R735.2 [文献标识码] A [文章编号] 1674-0742（2016）03（b）-0157-03 　　[Abstract] Objective To discuss the value of 128 slice spiral CT in diagnosis of gastric cancer. Methods The clinical data of 90 cases of patients with gastric cancer were collected， all patients received 128 slice spiral CT examination and pathological examination after operation， the accuracy rate of CT in diagnosis of gastric cancer and its TNM staging were counted and analyzed according to the postoperative pathological diagnosis. Results The diagnosis rate of 128 slice spiral CT for gastric cancer was 97.8%， and the detection rate of 128 slice spiral CT for gastric cancer in early stage was 71.4%， the detection rate of 128 slice spiral CT for gastric cancer in progressive stage was 100%， the diagnosis accuracy rates in T1 ，T2，T3 and T4 stages were respectively 71.4%， 76.9%， 86.7% and 90.0%， the diagnosis accuracy rates in N0， N1， and N2 stages were respectively 76.0%， 64.3% and 75.7%， and the diagnosis accuracy rates in M0 and M1 stages were respectively 92.5% and 91.9%. Conclusion 128 slice spiral CT can clearly show the shape， infiltration degree， lymphatic metastasis and distant metastasis of gastric cancer， and it is conducive to the diagnosis of gastric cancer and the choice of treatment Methods combined with the technology processing of multiplanar reconstruction and CT virtual endoscopy. 　　[Key words] 128 slice Spiral CT； Gastric cancer； TNM staging； Distant metastasis 　　胃癌是消化系统常见的恶性肿瘤，其诊断主要采用胃镜和钡餐造影两种方法。近期多层螺旋CT在胃癌的诊断中得到了广泛的应用，特别是多平面重建及CT仿真内镜等新技术可清晰显示病灶的形态、位置及癌灶的浸润深度及转移等情况，这对患者的术前评估具有极高的指导意义。
