恩平8000例体检妇女宫颈病变的的结果分析 　　[摘要]目的 了解恩平地区的妇女宫颈病变情况，探讨最佳的宫颈病变检出方式。方法 选取2005年12月～2016年4月我院的8000例妇科体检妇女作为研究对象，给予常规妇科检查，包括新柏氏液基薄层细胞检测（TCT）或巴氏涂片等宫颈脱落细胞学检查、高危型人乳头状瘤病毒检测，对于宫颈细胞学检查异常的妇女给予阴道镜和病理组织学检查，观察宫颈病变情况。结果 8000例妇科体检妇女中，宫颈病变4576例（57.2%）。4232例妇女接受TCT检查，异常147例（3.5%）；3768例妇女接受巴氏涂片检查，异常56例（1.5%）。TCT的灵敏度、阳性率高于巴氏涂片，差异有统计学意义（P0.05）。4576例阴道镜检查中，图像正常1934例，检出宫颈癌前病变（CIN）208例；图像异常2642例，检出CIN 2218例。3127例接受HPV检测，阳性532例（17.0%）。TCT检查异常患者的瘤变率（12.9%）高于HPV阳性患者（3.3%），差异有统计学意义（P0.05）。结论 在妇科常规体检中，HPV联合TCT检查可早期发现宫颈病变及宫颈癌，能够降低癌变发生率。 　　[关键词]人乳头状瘤病毒；宫颈病变；宫颈细胞学检查；妇科体检 　　[中图分类号] R711.74 [文献标识码] A [文章编号] 1674-4721（2016）08（a）-0130-04 　　[Abstract]Objective To get known the situation of women′s cervical lesions in Enping in order to find out the optimal approach to detect cervical lesion.Methods From December 2005 to April 2016，8000 women for physical examinations in our hospital were selected as research objects and were provided with regular gynecological examinations which included cervical smear cytological examinations containing Thinprep cytologic test （TCT） or Pap smear and high-risk human papilloma virus （HPV） detection.Those women whose cervical cytological examinations were abnormal were provided with colposcopy and histopathologic examination aiming at observing cervical lesions.Results Among 8000 women′s physical examinations，cervical lesions were in 4576 cases accounting for 57.2%.There were 4232 women underwent TCT examinations，and 147 （3.5%） cases were in abnormal.The rest cases were through Pap smear examinations，and 56 （1.5%） cases were in abnormal.The sensitivity and positive rate of TCT were both higher than those by Pap smear，with significant difference （P0.05）.Among 4576 cases with colposcopy，normal images were 1934 cases and the number of detecting cervical intraepithelial neoplasia （CIN） was 208 cases.Abnormal images were 2642 cases，and 2218 ones were detected with CIN.HPV detection was performed in 3127 cases，and 532 cases were in positive accounting for 17.0%.The rate of canc
