苏州地区黄褐斑216例临床剖析 　　[摘要]目的：探讨苏州地区黄褐斑患者的临床特点。方法：对笔者医院2013年3月-2014年3月确诊的216例黄褐斑患者的临床资料进行分析，并与中国不同地区的研究结果进行比较。结果：216例黄褐斑患者，男女之比为1：20.6， 30～39岁多见，皮肤类型以混合性皮肤、Ⅳ型皮肤为多，皮损部位以面上部较多见，伴发局部毛细血管扩张（38.9%）、颧部褐青色痣（31.5%）、日光性黑子（30.6%）、脂溢性角化（25.0%）、痤疮（17.1%）及雀斑（13.0%）等皮损，可能的致病因素主要为紫外线照射、内分泌因素、神经精神因素、美容不当、遗传因素等。结论：黄褐斑的发生与发展可能与多种内外因素相关，黄褐斑的某些临床特点可能存在着地域差异。 　　[关键词]黄褐斑；临床分析；苏州地区 　　[中图分类号]R758.4+2 [文献标志码]A [文章编号]1008-6455（2015）10-0054-03 　　Clinical analysis of 216 patients with melasma in Suzhou 　　SHANG Ying-bin，CHEN Fang-yuan，WANG Hong-qin 　　（Department of Cosmetic Dermatology，Suzhou Mylike Cosmetic Hospital， Suzhou 215000，Jiangsu，China） 　　Abstract： Objective To investigate the clinical features of patients with melasma in Suzhou. Methods The clinical data of 216 patients with melasma in our hospital from March 2013 to March 2014 were analyzed and compared with those seen in other areas in China. Results The male-to-female ratio in the patients was 1：20.6，with the age of most patients was found to be between 30-39 years.It was more common in mixed skin and Fitzpatrick skin types IV，with upper face distribution.Of the patients，38.9% had local telangiectasis，31.5% had nevus fusco-caeruleus zygomaticus，30.6% had solar lentigo，25.0%had seborrheic keratosis，17.1% had acne and 13.0% had freckles.The main risk factors of Melasma might include ultraviolet irradiation，endocrine factors，neuropsychic factors，improper use of aesthetic methods and genetic factors. Conclusion Various endogenous and exdogenous factors may be related to the occurrence and development of Melasma.Some clinical features of melasma might be different in different geographical areas. 　　Key words：melasma；clinical analysis；suzhou region 　　2013年3月-2014年3月于笔者医院皮肤美容科就诊的苏州地区216例黄褐斑患者的临床资料进行分析，以探讨黄褐斑的临床特点，现报道如下。 　　1 资料和方法 　　1.1研究对象 　　216例苏州地区黄褐斑患者，均符合2003年中国中西医结合学会皮肤性病专业委员会色素病学组拟定的黄褐斑诊断标准[1]。 　　1.2研究方法 　　采用统一设计的调查表，经知情同意后，由专业医师对黄褐斑患者进行问卷调查。调查表内容包括姓名、性别、年龄、职业、病程、可能的诱发或加重因素、皮损表现、伴发皮损、皮肤类型、伴随疾病及家族史等。 　　2 结果 　　2.1 一般资料
