Methods Since August 2002 -2012 in September during my hospital, 12 cases of different ages congenital elevated scapula were treated by Modified Woodward operation, postoperative neither line plaster or brace, combined with early postoperative active and passive combined rehabilitation exercise, the average age at the operation was 7.3 years , the minimum one was 3 years ,and the oldest one was 26 years, 3 years - 8 years of which seven cases, five cases of more than 8 years of age, 4 males and 8 females, the left side of involvement of the nine cases, the right side of the involvement of the three cases. All patients underwent bilateral scapula spiral CT examination, shoulders unequal height, difference of 1.7-4cm ranging from an average of 2.1cm, Limb function is limited outreach ranging from 60  -140  ,an average 89  . According to the the Cavendish classification method, II ， 2cases ， III,7 cases,IV, 3 cases.All the patients had other associated abnormalities. Most accompanied abnormalities were spinal column and ribs deformities. Results 12 patients were successfully followed up. The average follow-up period after operation was 2 yeas. The average scapular lowered was 2.1cm.Limb outreach function were improved to varying degrees, from an average of 89  increased to 145  , cavendish classification recovery by preoperative class II-IV, to the I-III level after,beautiful appearance and limb function has been greatly improved.No one had complicated of brachial plexus injury. Conclusions Modified Woodward operation for congenital elevated scapula,. not only made good efficacy in patients under the age of 8, can be also achieved good results for the patients over the age of 8, with simple an
