臺灣兒科醫學會第二二四屆學術演講會醫學倫理講座- -外賓介紹 演講時間：民國 104 年11月 15日 星期日( ) 14:20~15:10 演講地點：國防醫學院三樓 致德堂 演講主題：Early childhood adversity, toxic stress and the role of pediatrician 演講者：Prof. Robert Allan Shapiro 主持人：葉國偉副祕書長、宋文舉副教授 演講摘要： There is an extensive body of scientific literature that associates early life trauma and exposure to adversity with profound changes in brain development, gene expression, physiologic stress response and immune function. Young children are particularly vulnerable to the cumulative effects of toxic stress, defined as excessive or prolonged activation of the physiologic stress response systems in the absence of the buffering protection afforded by stable, nurturing relationships. These effects include lifelong mental and physical illness, learning difficulties, behavior problems and social maladaptation. Examples of early childhood adversity include child abuse and neglect, exposure to home violence, parental mental health or substance abuse problems, food insecurity and poverty. During the first few years of a child’s life, pediatricians are particularly well positioned to screen for and reduce toxic stress and to encourage safe, stable and nurturing home environments. Achieving this will require changes in pediatric practices, including the integration of an efficient and effective screening tool for social determinants of health and the development of active relationships with community partners. To optimize child development, referral and support services will be needed to strengthen parenting skills, promote positive social and emotional behaviors, ensure adequate housing and food, treat substance abuse and child and adult mental health problems. 演講者簡介： Robert Allan Shapiro, MD Director, Child Abuse Team/Mayerson Center for Safe and Healthy Children Professor, UC Department of Pedi
