骨代谢调节剂及其治疗骨关节炎的研究 摘 要 骨关节炎( osteoarthritis, OA)是一种骨关节退化性疾病，主要表现为关节软骨退变和软骨下骨硬化，引起患者关节疼痛、功能下降。近年来，骨代谢调节剂对其治疗作用成为研究热点并应用于临床。目前，OA的治疗以缓解关节疼痛和控制炎症为主，非甾体抗炎药(NSAIDs)通常是首选药之一，已经是公认OA症状的缓解药物，最常用的赛莱昔布具有良好的抗炎镇痛效果和胃肠道耐受性等优点，所以临床上用来治疗OA，但是其对OA引起退变的关节软骨修复作用不佳。其他方法如适当运动和物理疗法等保守治疗，除对症处理外均未能从根本上延缓OA进展，严重的患者需要关节置换。理论上最理想的治疗方法是防止、甚至逆转关节软骨形态的继续退行性变，为此人们对软骨细胞的增殖、分化、迁移及软骨基质的维持及降解作了大量的研究。骨代谢调节剂对OA关节具有保护作用，能够减缓骨关节炎软骨以及软骨下骨的破坏，稳定关节内环境，减轻OA样关节退变，阻止OA关节的破坏，此外，骨代谢调节剂联合非甾体抗炎药可以减少相关药物的使用量，降低单一用药的副作用，可能对OA的治疗具有更好的治疗效果。 关键词 ：骨关节炎；骨代谢调节剂；骨质疏松；关节 Research progresses on the effects of bone metabolic modulators on osteoarthritis Abstract Osteoarthritis is a degenerative disease of the joints, characterized mainly by degeneration of articular cartilage and subchondral bone sclerosis, resulting in joint pain and decreased function in patients. In recent years, bone metabolism modulators have become a research hotspot and used in clinic. At present, OA treatment to relieve joint pain and inflammation control, steroidal anti-inflammatory drugs (NSAIDs) is usually the preferred one of the drugs, is already recognized drug to alleviate OA symptoms, the most commonly used. Celecoxib has anti-inflammatory and analgesic effect and gastrointestinal tolerability advantages such as good, so it is used clinically for the treatment of OA. But on the repair of articular cartilage degeneration caused by OA effect is poor. Other methods, such as proper exercise and physical therapy, have not been able to radically delay the progression of OA except symptomatic treatment, and severe patients need joint replacement. The treatment method of the ideal theory is to prevent and even reverse articular cartilage morphology to degenerative changes, so people on the proliferation, migration and differentiation of chondrocytes and cartilage matrix degradation and maintain made a lot of research. Bone metabolic modulators have potential protective effects on osteoarthritis joints and it could prevent cartilage and subchondral
