PAGE PAGE 6 布加综合征的多层螺旋CT诊断 马秀华，吕富荣＊，尤云峰，肖智博，黄铀新，李越，张瑜 （重庆医科大学附一院放射科 重庆 400016） 摘要: 目的 探讨多层螺旋CT（MSCT）在布加综合征（BCS）中的诊断价值。方法 回顾性分析28例经DSA或手术证实的布加综合征MSCT影像特点。结果 BCS的MSCT特点为： = 1 \* GB3 ①CT平扫形态学改变：肝脏不规则增大15例，弥漫增大10例，未见明显改变3例； = 2 \* GB3 ②强化特征：花斑状强化型11例，周边强化型6例，中央强化型8例，不规则强化3例； = 3 \* GB3 ③CTA准确显示阻塞的部位及侧枝循环血管：肝静脉闭塞3例；下腔静脉狭窄闭塞25例，其中7例伴肝静脉闭塞；显示肝外侧枝循环16例，肝内侧枝循环2例，肝内外均有侧枝循环7例，3例无侧枝循环血管。结论 MSCT能准确直观反映布加综合征阻塞的性质、部位、范围、侧枝血管及其与周围器官的空间关系，能从形态及功能上反映肝脏损害程度。 关键词: 布加综合征; 体层摄影术，X线计算机 ;血管造影术 Multislice CT Diagnosis of Budd Chiari Syndrome MA Xiu_Hua, LV Fu_Rong, YOU Yun_Feng ,XIAO Zhi_Bo,HUANG You_Xin,LI Yue,ZHANG Yu(Department of Radiology，the First Affiliated Hospital, Chongqing University of Medical Sciences . Chongqing 400016 China) Abstract: Objective To evaluate the Multislice CT（MSCT）in the diagnosis of Budd-Chiari syndrome. Methods CT features of 28 cases with Budd-Chiari syndrome confirmed by DSA or operation .Results The MSCT feature in BCS : ①The morphology changing of CT plain scan: 15 cases of liver irregular increase ,10 cases diffuser increase, 3 cases no significant change. ② Enhanced feature: 11 cases enhance mottled, 6 cases periphery, 8 cases central, 3 cases irregular. ③ CTA accurately shows the location and obstruction of vascular collateral circulation: hepatic vein (HV)occlusion 3 cases; inferior vena cava( IVC) narrow occlusion 25 cases , and 7cases with HV occlusion. And collateral circulation outside the liver 16 cases, inside the liver 2 cases, 7 cases both inside and outside the liver, 3 cases have no collateral circulation . Conclusion Multislice CT can not only accurately and directly reflect the nature，the position ，the scope, the collateral circulation and the space relation, but also reflect the damage of liver from the shape and function.
