下颌骨喙突合并同侧颧弓骨折的手术治疗 魏信汉 谢卡 通讯作者：谢卡（1982-）硕士 主要从事口腔颌面外科研究。Email 通讯作者：谢卡（1982-）硕士 主要从事口腔颌面外科研究。Email （重庆医科大学附属永川医院口腔科，重庆 永川 402160） 【摘要】目的 探讨下颌骨喙突合并同侧颧弓骨折安全有效的治疗方案。方法 采用半侧冠状切口，在对颧弓骨折进行切开复位内固定手术的同时，对手术方式稍作改良，充分显露颧弓骨折区域后，将颧弓骨折块翻起，在其深面沿颞肌纤维走行方向向下继续分离寻找到喙突骨折块，并分离后摘除。结果 手术切口一期愈合，随访3-24个月，开闭口，下颌运动功能正常。结论 采用半冠状切口，治疗颧弓骨折的同时摘除下颌骨喙突骨折块，为手术治疗下颌骨喙突合并同侧颧弓骨折提出了一种新的并且安全有效的治疗方案。 【关键词】 下颌骨喙突； 颧弓；骨折；头皮半冠状切口。 Surgical treatment of zygomatic arch fractures associated with ipsilateral coronoid process Abstract: Objective Exploring an effective and safe therapeutic strategy for treating fractures of zygomatic arch associated with ipsilateral coronoid process fractures. Methods The patients underwent the open reduction and internal fixation of zygomatic arch fractures through the semi coronoid scalp incision. We then modified the operation. The area of zygomatic arch fractures was exposed adequately, and then the fracture fragments of zygomatic arch were turned up. We isolated and removed the coronoid process fracture pieces through direction of muscle fibers in the temporalis muscle. Results All the surgical incisions were healed by primary intention. At a 3-24 months follow up, the function of mouth opening and closing, and the movement of mandible were normal. Conclusion Using the semi coronoid scalp incision, we can treat zygomatic arch fractures and removed the coronoid process fracture pieces simultaneously, which provides an effective and safe therapeutic strategy for treating fractures of zygomatic arch associated with ipsilateral coronoid process fractures.
