布地奈德混悬液吸入治疗慢性阻塞性肺疾病急性加重期的临床观 【摘要】 目的 探讨雾化吸入布地奈德混悬液治疗慢性阻塞性肺疾病（COPD）急性加重期患者的疗效。方法 40例COPD急性加重期患者随机分成治疗组和对照组各20例，分别给予布地奈德混悬液及全身性应用糖皮质激素。观察2组治疗前后呼吸困难、肺功能、动脉血气变化及副作用的情况。结果 治疗组和对照组治疗前后的呼吸困难、肺功能、动脉血气变化均差异无显著性，雾化吸入布地奈德混悬液治疗组的副作用明显少于全身应用糖皮质激素的对照组。结论 雾化吸入布地奈德混悬液是治疗COPD急性加重期患者安全有效的方法，值得推广。 　　【关键词】 慢性阻塞性肺疾病；急性加重期；布地奈德 　　Clinlcal observation of nebulized budesonide suspension in treating acute exacerbations of chronic obstructive pulmonary disease 　　HU Guo-ping. 　　Department of Respiratory,Changzheng Hospital Zhabei Branch,Shanghai 200070,China 　　【Abstract】 Objective To Compare the effects of nebulized budesonide suspension and systemic glucocorticoids on acute exacerbation of chronic obstructive pulmonary disease.Methods Forty cases with acute exacerbation of COPD were randomly divided into two groups,treated with nebulized budesonide suspension or systemic glucocorticoids.To observe the changes and side-effects of decompensation,pulmonary function,blood gas before and after the treatment.Results The two groups had no significant differences on decompensation,pulmonary function and blood gas in the course.The treatment group with nebulized budesonide has fewer side-effects than that in the control group with systemic glucocorticoids.Conclusion Nebulized budesonide is worth spreading in treating acute exacerbation of chronic obstructive pulmonary disease as a safe and efficiency method. 　　【Key words】 chronic obstructive pulmonary disease;acute exacerbation;budesonide 　　应用全身性糖皮质激素治疗慢性阻塞性肺疾病（COPD）急性加重期患者，有益于加快病情缓解和肺功能恢复［1］。但与此同时，给COPD急性加重期患者带来的副作用却不能忽视。笔者采用雾化吸入布地奈德混悬液的方式进行治疗，以期既能达到相应的治疗效果又能减轻副作用的目的。现报告如下。 　　1 资料与方法 　　1.1 一般资料 按照慢性阻塞性肺疾病诊治指南［1］的临床诊断要求，选择2003年12月~2005年2月住院治疗的Ⅱ期COPD急性加重期患者40例，FEV1 30%~80%，FEV1/FVC＜70%。排除标准：呼吸衰竭需要机械通气、支气管哮喘、肺炎、气胸、心力衰竭等以及1个月内使用过激素治疗或不适宜使用激素治疗的患者。本组共40例，其中男26例，女14例，年龄63~78岁，平均72岁。随机分成雾化吸入布地奈德混悬液治疗组和全身应用糖皮质激素对照组各20例。2组年龄和基础肺功能比较差异无显著性。 　　1.2 方法 治疗组雾化吸入布地奈德混悬液2mg，2次/d。对照组静脉滴注氢
