* * * Step 1: Identify and describe the defect. It is helpful to show the defect rate over time. Refer back to page 3.10 of this manual to the worksheet you completed earlier. This provides some of the defect information you need. Team members should also be identified to establish accountability. Step 2: Determine where the defect is made / discovered. This is important information for later steps. Step 3: Detail the current standard procedure. Document each element / step in the operation where the defect occurred. Improvement is based upon this careful identification. Specify that the steps identified here are only those which probably contain the source of the error. Step 4: Identify deviations from standards. All errors and/or deviations from standard procedures point to where improvement is needed. * * * An error is any deviation from a specified manufacturing process. Errors can be made by machines or people and can be caused by previous errors that have occurred. All defects are created by errors. This means that if errors can be prevented, no defects will be created. By using Error-Proofing, the errors that cause defects are located and eliminated.
