浅析彩超对肝脏囊性病变的鉴别诊断 鸡西矿业总医院医疗集团平岗医院158165 摘要：目的研究分析彩色多普勒超声对肝脏囊性病变鉴别诊断的价值。方法此 次研究的对象是通过二维超声及彩色多普勒超声对临床和病理证实的16例肝脏 囊性恶性肿瘤、19例肝脓肿和30例肝囊肿患者。将其临床资料进行回顾性分析， 并根据声像图中病变的形态、囊壁厚度、病灶数目、囊内分隔、囊壁结节、血流 信号的性质及部位、囊内回声等，对三种病态的患者的声像图进行比较评分。结 果肝囊肿、肝脓肿及肝囊肿型恶性肿瘤的声像图变现出显著性差异(Plt;0.05)o 肝脏囊性恶性肿瘤患者中，囊壁及囊壁结节、囊内分隔、囊壁或分隔内侧测定的 血流信号和囊液透声性与其他两种病变患者比较存在显著性差异(Plt;0.05)。 而在病灶形态、病灶数目和边界方面无显著性差异(Pgt;0.05)o结论 二维超声 在肝脏囊性病变的鉴别诊断中起到一定作用，彩色多普勒超声在囊壁、分隔或乳 头肌上的血流信号检测方面对肝脏囊性恶性肿瘤诊断有很高的特异性。 关键词：彩色多普勒超声；肝脓肿；肝囊肿；肝脏囊性恶性肿瘤 Abstract： Objective To study the value of color Doppler ultrasonography in the differential diagnosis of hepatic cystic lesions.Methods the study was based on two dimensional ultrasound and color Doppler ultrasound in the clinical and pathological diagnosis of 16 cases of liver cystic malignant tumors, 19 cases of liver abscess and 30 cases of hepatic cysts.The clinical data were retrospective analysis, and according to the morphology of the ultrasonographic lesions, cystic wall thickness, number of lesions, cystic compartments, cystic wall nodules, blood flow signals in the nature and location, intracapsular echo, the sonogram of three kind of sick patients for score comparison.Results there were significant differences in the sonographic characteristics of hepatic cysts, hepatic abscess and hepatic cyst type malignant tumors (Plt;0.05) .Patients with hepatic cystic malignancy and separates in cystic wall and mural no dule, cystic and cystic wall or septa in side the determi nation of blood flow signal and cyst fluid through sound and the other two lesions were compared with the presenee of sign讦icant differenee (P lt; 0.05 ) .And there was no sign讦icant differenee in the shape of lesions, the number of lesions and the boundary (Pgt;0.05) .Conclusion two dimensional ultrasound in liver cystic lesions in the differential diagnosis plays a role, and color Doppler ultrasonography in cystic wall and septa and papillary muscle blood flow signal detection aspects of hepatic cystic malignancy diagnosis ha
