榄香烯注射液在子宫切口妊娠介入治疗中的应用 上海中医药大学附属曙光医院200021 【摘 要】目的：探讨榄香烯注射液经子宫动脉灌注结合动脉栓塞，治疗切口妊 娠的临床价值。方法：14例切口妊娠住院患者，入院1?3天内行双侧子宫动脉灌 注栓塞介入治疗。随机分为榄香烯组7例，经子宫动脉灌注榄香烯20-40ml；甲 氨蝶吟组7例，经子宫动脉灌注甲氨蝶吟50-100mg；双侧子宫动脉以明胶海绵 粉/碎粒、明胶海绵条栓塞。结果：介入术后两组患者均未出现子宫破裂大出血。 术后3天内观察血HCG变化。榄香烯组HCG下降＞50%6例，V5O%1例；甲氨 蝶吟组HCG下降＞50%4例，＜50%3例。结论：榄香烯区域灌注结合子宫动脉 栓塞治疗切口妊娠疗效确切，榄香烯是中药莪术的提取物，有行气破血抗早孕的 功效。可能在抑制胚胎细胞繁殖的同时，还促进局部血栓的形成，促使孕囊缺血、 坏死。 【关键词】子宫切口妊娠，榄香烯，区域灌注，动脉栓塞 The application of Elemene injection in the interventional treatment for uterine incisional pregnancy Shanghai Shuguang hospital affiliated with Shanghai University of T.C.M. 200021 Departme nt of radiology Gen Jian, Yao Ming-r ong, Hua ng Xue-ji ng, Lu Meng-ying, Zha ng Arvjun Abstract： Objective： To discuss the clinical value of intra-arterial Elemene injection infusion, combined with arterial embolization.Method： A total of 14 patients with uterine incisional pregnancy, who received intra-arterial infusion combined with arterial embolization within 1-3 days of admission, were studied. They were randomly assigned into two groups： patients in group Elemene (n二7) received intra-arterial infusion of Elemene injection(20?40ml).Patients in group Methotrexate (MTX, n=7) received intra-arterial infusion of Methotrexate (50-100mg) .Bilateral uterine arterial embolization was performed with gelfoam particles or gelfoam sponge.Result： Neither group experieneed rupture of uterus after treatment. The change of B-HCG was observed within 3 days of operation. Among the Elemene group, 6 patients exhibited a decrease in HCG gt;50%, and 1 exhibited a decrease lt;50%； among the MTX group 4 patients exhibited a decrease gt;50%, and 3 exhibited a decrease lt;50%.Conclusion： The method of intra-arterial Elemene injection infusion combined with arterial embolization in incisional pregnancy is effective. Elemene is an extract of Rhizoma Zedoariae . It has an effect of accelerating QI and blood circulation which could promote the termination of early pregnancy. It may inhibit the rep
