先天性髋关节脱位的X线分析 潘炳灿 刘英华 菏泽医学专科学校诊断教研室 菏泽 274000 中国 【摘要】 目的：探讨X线检查在先天性髋关节脱位早期诊断中的应用价值。方法：收集121例1周岁以内骨盆正位片，其中随访证实先天性髋关节脱位患儿21例，正常婴幼儿100例，患髋28例，正常髋关节214例，分别对其髋关节正位进行测量分析，项目包括帕金（Perkin）方格、髋臼角、h值、f值、Shenton线、Clave线。结果： 214例正常髋关节髋臼角：男21.90o±3.72 o 、 女 25.02 o±3.45 o；h值：男( 0.90 ±0.13) cm、女 (0.86±0.12 ) cm；f值：男(0.29±0.21)cm、 女(0.30±0.11)cm；Shenton线：Shenton线连续206例，不连续8例；髂颈线（Clave线）均连续；股骨头骨骺显示者均在帕金（Perkin）方格内下象限。28例患髋髋臼角：30.46 o±3.75 o，h值 (0.39±0.42) c m， f值(0.64±0.23 )cm，h＜f；Shenton线、髂颈线（Clave线）连续性中断；股骨头骨骺显示者均在帕金（Perkin）方格外上和（或）外下象限。结论：髋臼角、h值、f值、Shenton线、帕金（Perkin）方格等指标综合运用在CDH诊断中有重要意义。 【关键词】 先天性；髋关节脱位；X线诊断；婴幼儿 Congenital dislocation of the hip X-ray analysis Pan Bing-can Liu Ying-hua 【Abstract】 Objective: Explore X-ray examination in the early diagnosis of congenital dislocation of the hip application value. On the pelvic anteroposterior films is a separately measured analysis, project including Perkins box, angulus acetabulum, h, f value, Shentons line, Claves line. \ Results: 214 cases of hip angulus acetabulum normal size male is 21.90 o ± 3.72 o, women were 25.02 o ± 3.45 o; h value of M is (0.90 ± 0.13) cm, women were (0.86 ± 0.12) cm; f value: M ( 0.29 ± 0.21) cm, females (0.30 ± 0.11) cm; Shentons line: Shentons line 206 cases of continuous, discontinuous 8 cases; Clave line continuous; femoral epiphysis are within the inferior quadrant in the Perkins box.
