Pemberton手术治疗儿童发育性髋关节脱位疗效因素分析 冯太升1，李 明2，周士铭2，曹光彪2，米 爽2，周 进2（637000 四川　南充，南充市中医院骨科） [摘要] 目的　研讨Pemberton手术治疗儿童发育性髋关节脱位（developmental dysplasia of the hip,　DDH），前倾角（femoral neck anteversion，FNA）、髋臼指数（acetabular index,AI）、颈干角（collodiaphyseal angle,CDA）等因素对手术疗效的影响。方法　回顾性分析2005年1月至2010年1月采取Pemberton手术治疗儿童DDH 96例（118髋），其中男性17例（20髋），女性79例（98髋），平均年龄6.5（2～12）岁。术前双侧髋关节X片及三维CT重建扫描测量髋臼指数、前倾角及颈干角,术中行股骨截骨、髋臼造盖及关节囊修整，术后复查髋臼指数、前倾角及颈干角，并进行统计学分析。结果　96例平均随访42（18～60）个月。根据髋关节功能Mckay评分标准：优86髋（77.55%），良24髋（16.32%），可8髋（6.12%），差0髋（0%）（问题１：百分率如何算得？/118?），髋臼指数缩小（16.19±0.74）°，前倾角缩小（15.3±1.5）°，颈干角缩小（22.7±1.7）°，采取SPSS　17.0统计软件进行t检验，髋臼指数、颈干角、前倾角手术前后均有显著性差异(P0.01)；与术前正常侧相比，均无显著性差异（P0.05）。结论　儿童DDH行Pemberton手术治疗，髋臼指数、前倾角、颈干角是影响治疗效果的３个重要方面，髋臼指数、前倾角的矫正有利于髋臼成形及维持髋关节稳定，颈干角的矫正则有利于增加髋臼对股骨头的包容度；术中适当的进行关节囊修整重建，对于术后髋关节的稳定也非常重要。上述因素的正确处理对于DDH术后股骨头稳定复位及防止再脱位有重要作用。鉴于Pemberton手术治疗时常会涉及到其中的一个或全部，因此，术前准确评估，术中精确操作，对提高手术疗效意义重大。 [关键词]　发育性髋关节脱位；Pemberton手术；髋臼指数；前倾角；颈干角 [中图法分类号]　　　　　　　　　　　　　[文献标志码]　A [通信作者]　李　明， E-mail: Lm3180@163.com Analysis of Curative effect influencing factors of Pemberton acetabuloplasty for Developmental dysplasia of the hip in children （问题2：参照近年本刊文献，补充全部作者名及单位英文信息） ABSTRACT Objective: To evaluate the relationship in Pemberton acetabuloplasty for DDH among acetabular index, femoral neck anteversion, collodiaphyseal angle and articular capsule.
