呼吸ICU危重感染患者血清肌钙蛋白I浓度与APACHEⅢ评分及预后关系的研究 何远强 郑玉龙 周黎阳 万玉峰 刘淑 丁宗励 (1.江苏淮安市第二人民医院呼吸内科 淮安 223002） 　 [摘要]【目的】探讨呼吸ICU(RICU)危重感染患者血清肌钙蛋白(cTnI)浓度改变与急性生理学及慢性健康状况评分Ⅲ(APACHEⅢ)及预后的关系。【方法】检测120例入住RICU的危重感染患者的血清cTnI浓度,并对危重感染患者进行APACHEⅢ评分;分析血清cTnI浓度与APACHEⅢ评分及死亡率的关系。【结果】120例患者血清cTnI平均浓度为(0.25±0.12)ng/mL,存活组血清cTnI浓度(0.14±0.09)ng/mL,死亡组血清cTnI浓度(1.72±0.53)ng/mL,两组比较差异有显著性(P 0.01); 120例患者平均APACHEⅢ评分(48.6±18.7)分,存活组APACHEⅢ评分(37.6±22.2)分,死亡组APACHEⅢ评分(65.2±29.4)分,两组比较差异有显著性(P0.01)。血清cTnI浓度与APACHEⅢ评分呈正相关(r =0.7012,P 0.01),随各组血清cTnI浓度增高,APACHEⅢ评分值亦增高(F =28.69,P0.01),各组死亡率也随之增高(χ2 = 22.18,P0.01)。【结论】呼吸ICU危重感染患者血清cTnI浓度和APACHEⅢ评分明显增高,对判断病情及预后有一定的作用。 [关键词] 急性生理学及慢性健康状况评分Ⅲ; 肌钙蛋白; 危重病; 预后 Study on the relationship between serum troponin and APACHE Ⅲ scores,the prognosis of the patients in respiratory intensive care unit He Yuanqiang ( Department of Respiratory Medicine, the Huai，an Hospital Affiliated to xuzhou Medical college ,Jiangsu ,223002 China) [Abstract] 【Objective】To explore the relationship between acute physiology and chronic health evaluation Ⅲ (APACHEⅢ) scores and serum troponin concentration , prognosis of patients admitted to respiratory intensive care unit。【Methods】The APACHEⅢ scores of the patients were assessed. The serum troponin levels were measured in 120 cases, and the relationship between APACHEⅢ scores and serum troponin concentration, Mortality were analyzed. 【Results】The average APACHEⅢ score of the 120 cases was (48.6±18.7). The APACHE Ⅲ score(37.6±22.2) in the survival group was significantly lower than that (65.2±29.4) in the dead group,P 0. 01. The average serum troponin concentration of the 120 cases was (0.25±0.12) ng/mL. The troponin concentration in the survival group was(0.14±0.09)ng/mL, which was significantly lower than(1.72±0.53)ng/mL in the dead group, P 0. 01.
