If there are no local examples, the instructor may use the following incident: An employee needed to make a repair that was at a height out of his reach. Rather than use a ladder, the individual proceeded to step on a trash can. The cover of the trash can collapsed and the employee fell into the trash can. In his attempt to get out of the trash can, the employee injured his shoulder.
