Organizational Diagnosis Questionnaire
NOTE: This is NOT AC’s Change Readiness Survey. It is another survey that serves a similar purpose.
Rule 1
Any statement where more than 50% of the respondents score 3 or below is an identified strength of the organization.
Rule 2
Any statement where more than 50°/O of respondents score 4 or more should be identified as an area of potential weakness for the organization. The neutral point is included because organizations in a changing and competitive world need high levels of effectiveness. Neutral responses suggest something less than that.
Rule 3
Any statement where more than 30% score 5 or more should be identified as a weakness for the organization. It is worth noting that 10 to 15% dissatisfaction is common in employee surveys. While statements for which 10 to 15% or more score 5 or above cannot be ignored, this rule adopts a cutoff well above that Any weakness highlighted by Rule 3 is clearly something not to be ignored.
Organizational Diagnosis Questionnaire
This questionnaire is designed to help you to determine how well your own organization works in a number of areas. Assess the degree to which you agree or disagree with the following statements as they apply to you within your own department or section, using the seven-point scale and circling the appropriate number.

In answering the statements, be as honest as you can. All answers will be kept confidential. This is not a test, and there are no right or wrong answers. The only correct answer is what you decide yourself.
