PAGE PAGE 24 LEAVE OF ABSENCE AND TERMINATION PROCESS Background The leave of absence and termination process includes all employees who go on Sick Leave of Absence Coverage (“SLOAC”) or are terminated. Employees are entitled to SLOAC for maternity, authorized leave without pay as a result of temporary disability or illness, and Workers Compensation. Any optional or additional coverage over the basic coverage will be paid by the employee as NYCHA only covers basic health care for employees on SLOAC. Coverage for employees on SLOAC is to be terminated after all earned leave time has expired plus an additional four months. If an employee returns from SLOAC prior to the allotted time, their coverage continues without any changes. In termination cases, coverage stops when an employee goes off payroll. The process begins when the Employee Benefits Division receives the bi-weekly Worksheet to Support Payroll Control (“worksheet”) from Computer Services. The worksheet is the main source of information regarding SLOAC and termination for the Employee Benefits Division. It is generated based on the payroll record and lists all employees who have dropped or changed any payroll deductions (union dues, health insurance, life insurance, etc. ...). From this worksheet, a Benefits representative highlights all employees for whom coverage has been discontinued and hand writes the employees name and social security number on the Request Permission to Carry form (“128.011 form”). There are twelve employees listed on each form. Employee Benefits then sends the 128.011 form to the Records Control Division of the Personnel Department. It takes two to three days for the Employee Benefits Division to forward the 128.011 form to Records Control. This is a significant amount of time, especially when both divisions are part of the Personnel Department.
