YOUTH BUSINESS INTERNATIONAL: CATEGORIES ENTRY FORM 2012
Reminder of the basic Entry Criteria:
Business must be in profit.
Made equivalent of 12 months loan repayment and have no current arrears.
Aged 18 to 35 when received the loan.
Trading between 12 and 48 months.
Received true start up funds (or evidence that business was in development stage).
Access to a volunteer business mentor.
This Entry Form is for entries to EOY Categories only.

COMPLETING SECTIONS:
To reduce the work of our members, member CEOs can ask their entrepreneurs to fill in the Entry Form in draft, but the final submission must be written by member staff and checked and approved by the member CEO.
Member staff must complete Sections: 1, 2, 3, 8 and 10.
Entrepreneurs must complete Sections: 4, 5, 6, 7 and 9.
SECTION 1: BUSINESS SUMMARY (completed by member CEO)
In less than 50 words describe the entrepreneur’s business. For example: Organic tomato grower based in Kent, United Kingdom producing 4 tonnes of tomatoes per annum, employing 100 local people full time. Selling fresh tomatoes and tomato based canned products to supermarkets throughout the south of England, exporting 30% canned products to France and Belgium. Plans to export throughout Europe. Maximum 50 words.
