分娩镇痛在自然分娩初产妇中的应用效果 　　[摘要]目的探讨分娩镇痛在自然分娩初产妇中的应用效果。方法回顾性分析2018年1月～2019年7月在我院自然分娩的102例初產妇作为研究对象，根据选择分娩干预方式的不同分为对照组（n=51）和观察组（n=51）。对照组采取非药物性镇痛，观察组采取非药物性镇痛联合椎管内麻醉镇痛，比较两组产程进展、产后出血量、疼痛程度、apgar评分及妊娠结局。结果两组第一产程、第二产程及第三产程及总产程比较，差异无统计学意义（p0.05）;观察组产后出血量为（239.84±21.69）ml，少于对照组的（287.04±22.31）ml，视觉模拟量表（vas）评分为（5.06±0.69）分，低于对照组的（6.79±0.78）分，5min时apgar评分为（8.55±0.54）分，高于对照组的（8.31±0.50）分，10min时apgar评分为（8.95±0.47）分，高于对照组的（8.64±0.43）分，差异均有统计学意义（p0.05）;观察组不良妊娠结局总发生率（3.92%）低于对照组（17.65%），差异有统计学意义（p0.05）。结论采取分娩镇痛措施对自然分娩初产妇产程影响较小，且可减少产后出血情况，缓解产后疼痛，改善妊娠结局，值得临床推广应用。 　　[关键词]自然分娩;分娩镇痛;椎管内麻醉镇痛;初产妇;产程进展;妊娠结局 　　[abstract]objectivetoinvestigatetheapplicationeffectoflaboranalgesiainprimiparaewithnaturalchildbirth.methodsaretrospectiveanalysiswasperformedon102primiparouswomenwhogavebirthnaturallyinourhospitalfromjanuary2018tojuly2019.theyweredividedintocontrolgroup（n=51）andobservationgroup（n=51）accordingtothedifferentwaysofdeliveryintervention.inthecontrolgroup，non-druganalgesiawasused，andintheobservationgroup，non-druganalgesiacombinedwithspinalanesthesiawasadopted.theprogressoflabor，postpartumhemorrhage，pain，apgarscoreandpregnancyoutcomewerecomparedbetweenthetwogroups.resultstherewasnosignificantdifferenceinthefirst，second，third，andtotallabortimebetweenthetwogroups（p0.05）.thepostpartumhemorrhagewas（239.84±21.69）mlintheobservationgroup，lessthanthatinthecontrolgroupfor（287.04±22.31）ml.thevisualanaloguescale（vas）scorewas（5.06±0.69）points，lowerthanthatofthecontrolgroupfor（6.79±0.78）points.at5minutes，theapgarscorewas（8.55±0.54）points，higherthanthatofthecontrolgroupfor（8.31±0.50）points.at10minutes，theapgarscorewas（8.95±0.47）points，higherthanthatofthecontrolgroupfor（8.64±0.43）points，andthedifferenceswerestatisticallysignificant（p0.05）.thetotalincidenceofadversepregnancyoutcomesintheobservationgroupwas3.92%，lowerthanthatinthecontrolgroupaccountingfor17.65%，withstatisticaldifference（p0.05）.conclusionapplicationeffectoflaboranalgesicmeasureshaslesseffectonthelaborstagesinprimiparouswomenwithnatur
