Objectives:Thegoalwastodetermineifaddinginhaledbudesonidetostandardasthmatherapy
improvesoutcomesofpediatricpatientspresentingtotheemergencydepartment(ED)withacute
asthma.
Methods:Theauthorsconductedarandomized,double-blind,cebo-controlledtrialinatertiarycare,
urbanpediatricED.Patients2to18yearsofagewithmoderatetosevereacuteasthmawererandom-
izedtoreceiveeitherasingle2-mgdoseofbudesonideinhalationsuspension(BUD)ornormalsterile
saline(NSS)cebo,addedtoalbuterol,ipratropiumbromide(IB),andsystemiccorticosteroids(SCS).
Theprimaryoutcomewasthedifferenceinmedianasthmascoresbetweentreatmentgroupsat2hours.
Secondaryoutcomesincludeddifferencesinvitalsignsandhospitalizationrates.
Results:Atotalof180patientswereenrolled.Treatmentgroupshadsimilarbaselinedemographics,
asthmascores,andvitalsigns.Atotalof169patients(88BUD,81NSS)wereassessedfortheprimary
outcome.Nosignificantdifferencewasfoundbetweengroupsinthechangeinmedianasthmascoreat
2hours(BUD3,NSS3,p0.64).Vitalsignsat2hourswerealsosimilarbetweengroups.Fifty-six
children(62%)wereadmittedtothehospitalintheBUDgroupand55(62%)intheNSSgroup(differ-
ence0%,95%confidenceinterval
