Onecontestedcontraindicationtomedialunicompartmentalkneearthrosty(UKA)has
beenstatusofthepalofemoraljoint.SurgeonshaveavoidedUKAwhenthepalofemo-
raljointhasradiographicevidenceofarthriticchanges.However,recentstudiesadvocate
ignoringpalofemoraljointstatuswhenconsideringUKA.Thepurposeofthisstudywas
tocomparethefailurerateof-bearing,medialUKAinpatientswithandwithout
preoperativeradiographicevidenceofpalofemoraljointdegeneration.Preoperativera-
diographsfromarandomselectionof503patients(638knees)treatedwithUKAforan-
teromedialosteoarthritiswereassessedbyanobserverblindedtoclinicaloutcome.The
palofemoraljointwasgradedusingthemodifiedAltmanclassificationfrom0to3with
0beingnoevidenceofchangesand3beingsevere,andidentified396grade0,168grade
1,65grade2,and9grade3knees.At1-to7-yearfollow-up,therehavebeen17revisions
foroverallsurvivorshipof97.3%.Kan-Meierysispredicted97.9%survivalinknees
withpalofemoraljointdiseaseand93.8%survivalinkneeswithoutpalofemoraljoint
diseaseat70months(P.1).Failurerequiringrevisionoccurredin3.5%(14/396)ofgrade
0knees,1.2%(2/168)ofgrade1,1.5%(1/65)ofgrade2,and0%(0/9)ofgrade3.No
