Background:Complexacuteproximalhumeralfracturesmayrequireprostheticreplacementofthe

proximalhumerus.Reverseshoulderarthroplasty(RSA)hasbeensuggestedasanalternativetohemiar-

throplastyinthemanagementofsuchfractures.ThisstudycomparedthefunctionaloutcomesofRSA

withhemiarthroplastyinpatientswithacuteproximalhumeralfractures.

Materialsandmethods:AllpatientswhounderwentRSAorshoulderhemiarthroplastyforacute

proximalhumeralfracturesbetweenJanuary1,1999,andDecember31,2010wereidentiﬁedfromThe

NewZealandJointRegistry.Baselineinformation,operativecharacteristics,andpostoperativeoutcomes

(OxfordShoulderScore[OSS]at6monthsand5years,revisionrate,andmortalityrate)wereexamined

andcomparedbetweenthestudygroups.

Results:Duringthestudyperiod,55patientsunderwentRSAand313underwentshoulderhemiarthro-

plastyforacuteproximalhumeralfractures.Comparedwithhemiarthroplastypatients,RSApatients

weresigniﬁcantlyolder(meanage,79.6vs71.9years;P.001)andmoreoftenwomen(93%vs78%,

P¼.013).The6-monthOSSwas28.1forRSAand27.9forhemiarthroplasty,whichwasnotsigniﬁcantly

different(P¼.923);however,theRSAgrouphadasigniﬁcantlybetter5-yearOSSthanthehemiarthro-

plastygroup(41.5vs32.3;P¼.022).Therewasnosigniﬁcantdiffer
