ServiceAgreement

ServiceDescription:ThisserviceagreementisbetweentheServiceProviderandtheServiceRecipient.TheServiceProvideragreestoprovidetheServiceRecipientwitharangeofservices,includingbutnotlimitedto[listofservicestobeprovided].TheServiceRecipientagreestopayfortheseservicesinaccordancewiththetermsandconditionssetoutinthisagreement.

ServicePeriod:Theserviceswillcommenceon[startdate]andcontinueuntil[enddate]oruntilterminatedearlierinaccordancewiththetermsofthisagreement.

ServiceFees:TheServiceRecipientagreestopaytheServiceProviderfortheservicesprovidedinaccordancewiththefollowingschedule:

1.InitialPayment:TheServiceRecipientshallpaytheServiceProvideraninitialpaymentof[amount]uponsigningthisagreement.

2.MonthlyPayments:TheServiceRecipientshallpaytheServiceProvideramonthlypaymentof[amount]onthefirstdayofeachmonthduringtheserviceperiod.

3.LatePayments:Ifapaymentisnotreceivedbytheduedate,alatefeeof[amount]perdayshallbecharged.

PaymentMethod:Allpaymentsshallbemadeby[paymentmethod],withallpaymentsbeingnon-refundableandnon-negotiable.

ServiceLevelAgreement:TheServiceProviderguaranteesaminimumlevelofservicequalityandavailabilityasdetailedbelow:

1.ResponseTime:TheServiceProvidershallrespondtoallrequestsforassistancefromtheServiceRecipientwithin[responsetime].

2.ResolutionTime:TheServiceProvidershallresolveallissuesreportedbytheServiceRecipientwithin[resolutiontime].

3.SystemAvailability:Thesystemshallbeavailable[systemavailabilitypercentage]ofthetime.

ServiceTermination:Thisserviceagreementmaybeterminatedbyeitherpartyunderthefollowingcircumstances:
