2014-1-24主任加工版 老年急性非结石性胆囊炎早期手术疗效分析 李建平 （新都区石板滩镇卫生院，四川 成都 610500） [摘要]目的：探讨老年急性非结石性胆囊炎(acute acalculous cholecystitis，AAC)的临床特点及早期手术的疗效。方法：以我院2009年1月至2012年12月收治的老年AAC 患者，共41例为AAC 患者，共42例为入院2～3 d内早期老年，老年AAC并发症，改善预后 [关键词非结石性胆囊炎[中图分类号]R [文献标志码]A doi:10.11851/j.issn.1673-1557.2014.01.0 Effect Analysis of Early Surgery in the Elderly Patients with Acute Acalculous Cholecystitis Li Jianping (Xindu Shibantan Institutes of Health,Sichuan Chengdu 610500,China） [Abstract]Objective:To investigate the clinical features of the elderly patients with acute acalculous cholecystitis and the effect of early surgery for them.Methods:41 elderly patients with acute acalculous cholecystitis were the early therapeutic group from January in 2009 to December in 2012;and 42 elderly patients with acute acalculous cholecystitis were elective opportunity surgery group ( control group) at the same time.The patients with mild symptoms in early therapeutic group had surgical treatment in 2～3 days after symptoms were alleviated,the patients with severe symptoms in early therapeutic group had immediately surgical treatment.But the patients with mild symptoms in control group had surgical treatment after symptoms were alleviated,the patients with severe symptoms had surgical treatment when the symptoms were alleviated for 3～5 days,the patients with severe symptoms in control group had immediately surgical treatment when the symptoms did not have be alleviated.Results:The incidence rate of gallbladder gangrene and perforation in early therapeutic group were lower than in the control group.The incidence rate of postoperative complications in therapeutic group was lower than in the control group.The differences had statistically significant.Conclusions:The early surgery can effectively reduce the incidence rate of postoperative complications and improve prognosis for the elderly patients with acute acalculous cholecystitis.
