利用野战介入方舱救治急性心肌梗死的可行性研究

[摘要] 目的探讨灾难现场使用自主研制的新型野战介入方舱实施经皮冠状动脉介入
(PCI)治疗救治急性心肌梗死(AMI)的可行性。方法12只实验犬随机分为实验组和对照组(每
组a=6)，两组均行选择性冠状动脉造影(SCA)并建立AMI模型。造模成功后1h，实验组于方舱
内行急诊PCI术，对照组仅给予单纯药物治疗。观察两组治疗效果。在动物实验的基础上，开展
边远山区冠心病患者现场介入诊治，实地检验方舱的救治能力，并与同时期我院导管室介入手术

效果进行比较。结果动物实验PCI手术成功率100％，耗时20—34(27．0±6．0)min。术后3
复查SCA，实验组栓塞血管再通，支架内无血栓形成，对照组栓塞血管无再通。实验组心肌梗死
区／危险区(IS／AAR)为(24．2±5．7)％，对照组IS／AAR为(49．4±8．3)％，两组比较差异有统计
学意义(P0．05)。临床救治手术成功率100％，SCA术耗时17—34(26±6)min，PCI术耗时
33～51(38±6)min，方舱内救治效果与导管室比较差异无统计学意义(P0．05)。结论在灾
难现场，应用野战介入方舱开展PCI救治AMI患者是安全、可行的。
