中国超声医学杂志 2909年 11月 第 25卷第 11期 ChineseJUltrasoundMedVol25 No1l November2009 乳头状肾癌和嫌色细胞癌 的超声造影表现 黄备建 俞清 周盛 金赘杰 韩红 丁红 王文平 摘 要 目的 探讨超声造影在乳头状 肾癌和嫌色细胞癌 中的表现 。 方法 对 22例 22个经手术病理证实为乳头状肾癌和嫌色细胞癌的常规超声及超声造影声像 图进行 回顾性分析。常规超声 观察 肾占位大小 、回声、边界 、有无彩色血流信号。超声造影观察病灶 的增强方式和增强时相 ，包括起始 时间、达峰值 时间、 消退时间及病灶内部结构 的增 强表现 。 结果 本组 22个肾 占位经手术和病理证实乳头状 肾癌 13个和嫌色细胞癌 9个 。22个肿块最大直径范 围 i．I～14．2cm。常 规超声显示实质性肿块 17个 ，囊实性肿块 5个 ；18个病灶 内发现彩色血流信号 ，4个未显示彩色血流信号 。超声造影显示 22个 病灶均见增强 ，同步增强 13个 ，缓慢增强 9个 ，其延迟时间 2～4S，达峰值和实质期增强程度始终低于肾皮质 ；实质期快速消 退 16个 ，同步消退 6个 ，共 11个肿块周边显示假包膜 。 结论 缓慢增强和全期低增强对诊 断乳头状 肾癌和嫌色细胞癌有较大的帮助 。 关键词 超声造影 乳头状肾癌 嫌色细胞癌 诊断 TheM anifestationofContrast—enhancedUltrasOnography inRenalPapillaryCarcinomaandChromophobeCarcinoma HuangBeijian，YuQing，ZhouSheng，etal DepartmentofUltrasound，ZhongshanHospitalofFudanUniversity，Shanghai 200032 China Abstract：Objective Toinvestigatethemanifestationofcontrast—enhancedultrasonography (CEUS)inrenal papillarycarcinomaandchromophobecarcinoma．M ethods W eretrospectivelyanalyzedtheuhrasonogram ofroutine ultrasoundandCEUSin22CaSeSofrenalpapillarycarcinomaandchromophobecarcinomawhichwereprovedbyoper— ationandpathology．Thesize，echo，boundaryandcolorDopplerflow imaging (CDFI)signalofrenallesionswere observedbyroutineultrasound．ThenCEUS showed themodalityandphasesofenhancement，including thearrival time，thepeak，thewashouttimeand theappearanceofinternalstructures．Results Among the22casesofrena[1e— sionsprovedbyoperationsandpathololgy，therewere13casesofpapillarycarcinomaand9casesofchromophobecar cinoma．Themaximum diameterrangeofthe22renallesionswasfrom 1．1tO 14．2cm．Routineultrasoundshowed 17 casesofparenchymatousmassesand5easesofcysticmasses．CDFIsignalwasobservedin 18eases，andother4eases didnotshow anybloodstream signa1．CEUSshowedenha
