维普资讯 上 海 交 通 大 学 学 报 (医 学 版 ) Vo1．28 No．5 May2008 JournalofShanghaiJiaotongUniversity(MedicalScience) ·585 · 文章编号：0258—5898(2008)05—0585—03 -论 著 - 血清 S一100B蛋 白与碱性髓鞘蛋 白评估重型脑外伤预后的 对 比研究 李云峰 ， 李 宁， 王 健， 卞留贯 ， 孙青芳， 沈建康 (上海交通大学 医学院瑞金医院神经外科 ，上海 200025) 摘 要 ： 目的 探讨重型脑外伤患者血清 S-100B蛋 白和碱性髓鞘蛋 白(MBP)浓度在预后评估 中的价值 。方法 2006年 1月一 12月重型脑外伤患者 40例 ，患者均于外伤后 12h内抽取血清 ，分别应用单克隆双位点放免法和酶联免疫检测分析测定血清 S-100B蛋 白和 MBP浓度 ，以同期随机选取的 15名健康体检者作为正常对照，并结合格拉斯哥预后评分进行分析。设定分界值 和绘制 ROC曲线，比较 S-100B蛋白与 MBP评估预后的敏感性和特异性 。结果 重型脑外伤患者伤后血清 S-100B蛋 白和 MBP 浓度均显著高于正常对照组 ；重型脑外伤不同预后组之间 S-100B蛋 白和 MBP浓度存在显著差异。评估预后 的特异度和敏感度 在 S-100B蛋 白分别为91％和72％，MBP分别为63％和61％；ROC曲线显示 ，S-100B蛋 白对预后评估 的敏感性和特异性较 MBP 更强 。结论 伤后血清 S-100B蛋白浓度对评估重型脑外伤预后具有较高的特异性和敏感性，因此可作为临床上评估重型脑外伤 预后 的一种可靠方法。 关键词：S-IOOB蛋 白； 碱性髓鞘蛋 白； 重型脑外伤 ； 预后 中图分类 号 ：R651．15； R449 文献标志码 ：A ContrastanalysisofS·100B and myelinbasicprotein in outcomeprediction in patients withsevereheadinjury LIYun—feng，LINing，WANGJian，BIANLiu—guan，SUNQing—fang，SHENJian—kang (DepartmentofNeurosurgery，RuijinHospital，SchoolofMedicine，ShanghaiJiaotongUniversity，Shanghai 200025，China) Abstract： Objective ToinvestigatetheroleofS-100Bandmyelinbasicprotein(MBP)inoutcomepredictioninpatients withsevereheadinjury． Methods From January2006toDecember2006，40patientswithsevereheadinjury were involvedintothisstudy．Serum samplesofeachpatientwereobtainedwithin12hoursafterhead injury，andseurm concentrationsofS-100B and MBP were measured． Fifteen healthy controlswererandomly selected and analysiswas conductedwithGlasgow outcomescale．Thecutpointwassetandreceiveroperatingcharacteristiccurveswereobtained．The sensitivityandspecificityofS-100BandMBPinoutcomepredictionwerecompared． Results Theseurm concentrationsof S-100BandMBPinpatientswithsevereheadinjuryweresignificantlyhigherthanthoseofth
