从肝论治突发性耳聋的理论及临床探讨毕业论文.doc

提  要  目的：通过对 2010-2011 年度本科室门诊及住院突发性耳聋病人资料的统计分 析，对治疗前后的症状、体征的改善情况的对比，对影响疗效的相关因素进行了统计 分析，进一步探讨中医治疗突发性耳聋的理论内涵和更有效的方法。方法:首先对 2010-2011 年度本科室门诊及住院的突发性耳聋病人资料进行粗略的统计，观察其各 季节发病率、辨证分型所占的比例，并收集符合条件的肝火上扰型 60 例患者的临床 资料，将其分为两组，治疗组 30 例患者给予清肝通窍汤加减合用舒血宁治疗，对照 组 30 例给予通窍活血汤加减合用舒血宁治疗，观察两组治疗前后的疗效。结果:分析 发现突发性耳聋多发于春季，发病与肝关系密切。治疗组和对照组对听力的损失、眩 晕均有效，治疗组疗效优于对照组。治疗组总有效率是 73.3%，对照组总有效率是 53.3%，经统计分析（P＜0.01）两组差别有统计学意义。在治疗中两组患者没有发生 不良反应事件。结论：突发性耳聋发病多与情志有关，且春季发病率较高，其发病机 理复杂，涉及中医四时阴阳理论，且与肝关系密切。清肝通窍汤结合舒血宁静脉点滴 通过清肝泻火通窍，疏通经络，改善内耳血液循环，提高内耳的供血供氧，从而有效 的提高听力，改善伴随症状，促进病情的恢复，能有效治疗突发性耳聋及其伴随症状， 其疗效优于予以通窍活血汤加减合用舒血宁组的疗效。对耳聋程度越轻的患者疗效越 好，耳聋不伴眩晕者疗效优于伴见眩晕者。年龄、性别及伴有耳鸣等因素与药物疗效 无明显关系。  关键词  春季；突发性耳聋；暴聋；肝火上扰 The treatment of sudden deafness from liver theory and clinical discussion Speciality：E.N.T.of TCM Author：Wang Yun Tutor：Liang Jun Wei Abstract Objective: Through to the 2010-2011 year undergraduate course room outpatient service and hospital sudden deafness patients statistical analysis, Before and after treatment of the symptoms, signs of improving conditions of comparison, the related factors to affect curative effect the statistics and analysis, To further explore Chinese medicine treatment of sudden deafness theoretical connotation and more effective method.
