三分支支架血管术中植入及主动脉腔内补片技术在治疗 A 型主动脉夹层中的应用 研究生: 谢先标  导 师: 陈良万  教 授  摘  要  目 的：总结三分支支架血管术中植入及应用主动脉腔内补片技术成形主动 脉瓣治疗 StanfordA 型主动脉夹层的临床经验。 方 法：2009 年 7 月至 2011 年 03 月，我科对 45 例 Stanford A 型主动脉夹 层患者行升主动脉人造血管加三分支支架血管术中植入术，手术均在全身麻醉、 深低温停循环和选择性脑灌注下进行。单纯行升主动脉人造血管置换+三分支支 架血管术中置人术 11 例，行主动脉瓣成形+升主动脉人造血管置换+三分支支架 血管术中置人术 28 例，BENTALL+三分支支架血管术中植入术 5 例；BENTALL+三 分支支架血管术中植入+冠状动脉旁路移植术 1 例。 结 果：全组无手术死亡，本组体外转流时间：（128-310）min，平均（195.65 ±36.95）min；心肌阻断时间：（62-150）min，平均（89.05±18.06）min；低 流量脑灌注时间：（15-40）min，平均（30.75±6.64）min；停循环时间：（2-11） min，平均（4.86±2.18）min；术后胸腔及心包积液引流量：200～1550（ml）， 平均：684±452（ml），主动脉瓣成形者术后复查心脏彩超均未见明显主动脉瓣 返流，全组无术后再次开胸止血和永久性脑损害病例。住院期间死亡 3 例，1 例 为术后第一天腹主动脉夹层破裂，1 例为术后肾功能衰竭，1 例为术后严重肺部 感染，目前 42 例患者定期门诊随访，无死亡和需要再次手术病例。 结 论：三分支支架血管术中植入治疗 Stanford A 型主动脉夹层较传统外科 手术方法具有操作简易、手术时间短、术中术后出血量少及术后并发症少等特点; 主动脉腔内补片技术成形主动脉瓣能提高成形成功率及成形效果;深低温停循环 选择性脑灌注能有效地预防神经系统的并发症。 关键词：主动脉夹层；支架；外科治疗 4 Triple-Branched Stent Graft Implantation of Vascular Surgery and Aortic Valvuloplasty In The Treatment of Stanford –TypeA Aortic Dissection Postgraduate：Xie Xianbiao  Advisor：Chen Liangwan  Professor  【Objective】To summary the clinical experience of triple-branched stent graft implantation of vascular surgery and aortic valvuloplasty in the treatment of Stanford type A aortic dissection 【 Methods】Between July 2009 and March 2011, 45patients with acute Stanford type A aortic dissection underwent extensive primary repair of the thoracic aorta by means of ascending aorta replacement combined with triple-branched stent graft ,the operations were all performed under general anesthesia, were carried out under deephypothermia with circulatory arrest and selected cerebral perfusion. artificial blood vessel replacement and total arch grafting with triple-branched stent graft was carried out in 11 cases, aortic valvuloplasty+ artificial blood vessel replacement and+total arch grafting with triple-branched stent graft was carried out in 28 cases .BENTALL and total arch grafting with triple-branched stent graft was
