桂林市HIV感染者/病人配偶间HIV传播状况 及影响因素的调查研究 张振开 邓燕 文小青 陈伟 周芸 【摘要】 目的 了解HIV感染者/病人配偶间HIV传播的状况及影响因素，为政府制定有效的艾滋病防控政策和策略提供科学依据。方法 筛查2005-2010年所有桂林户籍的HIV确证阳性且有配偶的感染者或病人，对其个案调查信息资料中配偶HIV抗体检测结果和高危行为进行调查和核实，选择其中一方没有静脉吸毒、多性伴、有偿供血和输血等HIV感染的危险行为史为影响因素调查对象，对配偶HIV双阳组和单阳组HIV传播的相关因素进行调查、核实与分析。结果 共有345对HIV双阳或单阳配偶，HIV双阳率72.8%（251/345），HIV单阳率27.2%（94/345）。345对配偶中有明确配偶间传播130对，传播率37.7%，其中男传女103例，占79.2%，女传男27例，占20.8%。双阳组传播方和单阳组先感染方在年龄、安全套使用率、CD4细胞计数和病毒载量等因素方面差异有统计学意义（P＜0.05）。结论 桂林市配偶间HIV性传播率较高，影响配偶间HIV性传播的主要因素为安全套使用率低、年龄大、感染者体内CD4细胞计数低、病毒载量高。 【关健词】艾滋病；配偶；性传播；影响因素 The survey on HIV infection situations and influencing factors between spouses in Guilin City. Guilin center for Disease Control and Prevention, 541001,China. 【Abstract】 Objective To investigate the status and influencing factors of the HIV spousal transmission and to provide scientific evidence for HIV prevention and control strategy. Methods The HIV infect status of 345 couples HIV positive or patient in Guilin from 2005-2010 were investigated. The spouse of which one was HIV positive and the other had no known risk behavior such as drug injection, mufti-sexual partners, history of paid blood donation, blood transfusion and so on were recruited. All of the participants were investigated on suspected risk factors and laboratory tests for HIV antibody, CD4 lymphocytes and viral load. All of the data were analyzed with SPSS 11.5 statistical software. Results The HIV positive rate of both spouses was 72.8%(251/345) , the HIV infection rate of sexual transmission definitely was 37.7%(130/345). The age, CD4 cell count, condom use rate and HIV viral load in the case group were significantly different when compared to the control group（P＜0.05). Conclusions The infection rate of HIV sexual transmission between spouses was high.
