婴儿肝炎综合征血清内皮素水平检测及意义_临床医学论文 婴儿肝炎综合征血清内皮素水平检测及意义_临床医学论文 作者：史胜平 陈卫珍 滕小军 【关键词】 内皮素, 　　【摘要】 目的 探讨婴儿肝炎综合征（infantile hepatitis syndrome，IHS）患儿血清中内皮素（endothelin1，ET-1）的变化。 方法 采用放射免疫法检测56例IHS患儿血清中ET-1的浓度，同时测定其肝功能水平。 结果 急性期组血清ET-1水平显著高于对照组［（98.78±21.65）ng/L vs（55.81±14.32）ng/L，0.05］。好转组［（68.85±18.64）ng/L］及痊愈组［（57.70±14.38ng/L）］与对照组之间差异均无显著性（Pgt;0.05）。与急性期组相比，好转组及痊愈组的血清ET-1水平均显著降低（0.05）。重症组［（112.59±24.67）ng/L］及胆管闭锁组［（117.2±9.46）ng/L］血清ET-1水平均显著高于一般组［（86.36±15.33）ng/L，0.05］。IHS急性期患儿血清ET-1水平与ALT呈高度正相关（r=0.56，0.01），而与STB无明显相关性（r=0.34，Pgt;0.05）。 结论 ET-1血清水平的检测可以作为IHS诊断及评估IHS严重程度的一项参考指标。IHS血清ET-1水平异常升高，应高度怀疑胆管闭锁可能。 　　关键词 肝炎 内皮素 血清 婴儿 综合征 　　Serum level of endothelin in patients with infantile hepatitis syndrome 　 　　【Abstract】 Objective To investigate changes of endothelin1（ET-1）levels in infants with infantile hepatitis syndrome（IHS）.Methods Serum ET-1levels in20normal infants and56IHS patients were detected using ra-dioimmunoassay and their ALT and STB levels were also examined.Results Serum ET-1levels in acute stage of IHS patients were significant higher than that in controls［（98.78±21.65）ng/L vs（55.81±14.32）ng/L，（0.05）］.There was no markedly difference of ET-1serum concentrations between remission group（68.85±18.64ng/L）and healing group（57.70±14.38ng/L）and cnotrols（Pgt;0.05）.In comparison with acute stage group，ET-1serum levels in remission and healing group were significantly decreased（0.05）.Serum levels of ET-1in extra-hepatic biliary atresia group（117.2±9.46ng/L），as well as in severe group（112.59±24.67ng/L），were markedly higher than that in general group（86.36±15.33，0.05）.Serum levels in acute stage were significantly positively correlated with ALT levels（r=0.56，0.01），but not STB（r=0.34，Pgt;0.05）.Conclusion Detection of ET-1serum levels in patients with IHS may be a useful index for its diagnosis and evaluation of its severity.With unex-pected high ET-1serum levels，pediatric patients with IHS may be susceptible to extrahepatic biliary atresia. 　　Key words hepatitis
